阿尔茨海默病患者图形和汉字短时记忆的40Hz脑电的研究

【摘要】 目的:研究正常老年人和阿尔茨海默病患者在汉字记忆与图像记忆事件刺激状态下的40Hz事件相关电位(40HzERP)的变化规律和脑电非线性动力学参数在认知过程研究中的作用,探讨阵发性40HzERP和非线性动力学参数在阿尔茨海默病诊断中的意义和应用。方法:采集正常老年人和AD患者在安静闭眼、安静睁眼、汉字记忆、图像记忆时的16个导联40Hz脑电信号,选取20秒无伪差的40HzERP进行分析,分别计算安静闭眼、安静睁眼、汉字回忆、汉字配对、汉字再生、图形回忆、图像再生七种状态下40 Hz ERP的发放的总面积(TA)、总时间(TT)、关联维数(D2)和复杂度(Ck)等非线性动力学参数的变化情况。结果:1、正常人在安静状态下40HzERP的发放率低,无脑区优势和偏侧优势。2、正常人在汉字和图像记忆状态下40HzERP发放明显增加,较安静闭眼、安静睁眼的状态下的40HzERP的TA、TT、D2均有明显的改变,Ck与安静状态相比无明显改变,其中TT、D2与MMSE评分有相关性。3、正常人在记忆状态下40HzERP的各参数存在区域优势和偏侧优势。在汉字记忆中以Fp1导联为主,在图像记忆以P3导联为主。4、AD患者组与正常人群组相比,在安静状态下和记忆状态下40HzERP的发放明显减少,各脑区,各导联的TA、TT、D2明显降低,Ck无明显变化,并且去无区域优势和偏侧优势。结论:1、40HzERP的非线性动力学参数中TA、TT、D2在安静状态下和记忆状态下,存在明显差别;可以作为描述40HzERP的特征性参数使用;汉字和图像记忆事件可以作为检测高级脑机能的刺激事件。2、AD患者的40HzERP较正常人下降明显,且无脑区差别和偏侧优势,40HzERP的非线性动力学参数中,TA、TT、D2可用于AD的诊断。

【Abstract】 Objective: To study the role of changing and eeg non-linear dynamics parameters about the normal aging and Alzheimer disease(AD)who were stimulated with Chinese characters memory and image memory events on the condition of 40Hz event related potential (40HzERP) in cognitive process. We investigate the significance and application of paroxysmal 40HzERP and eeg non-linear dynamics parameters in Alzheimer’s disease diagnosis.Methods: We collected 16 standard leads of 40Hz eeg of the normal aging and AD patients in quiet close eyes, quiet open eyes,Chinese characters memory and image memory,selected 20 seconds 40HzERP without faise diference and analysis, and then separately calculated the variation of non-linear dynamic,s parameters about the releasing total area(TA) , the total time (TT), the correlation dimension (D2) and complexity (Ck) on 40 HzERP on seven conditions ,that is quiet, quiet open and close eyes memories, Chinese characters, the Chinese characterregeneration, graphics pairing memories, image regeneration.Results:1.On quiet condition, the rate of 40HzERP releasing of normal aging is low ,and no encephalic region advantages and lateralization advantage.2. On Chinese characters and normal image memory conditions, the 40HzERP releasings of normal aging increased significantly, and have more obviously change than TA, TT, D2 on quiet close eyes and quiet open eyes conditions. Compared with quiet condition ,Ck has no significant changes, and TT、D2 have dependablity with MMSE rating .3.On memory conditions,the parameters of 40HzERP of normal aging exist encephalic region advantages and lateralization advantage. On Chinese characters memory, Fp1 standard leads is primary ,on image memory, P3 standard leads is primary.4. On the quiet condition and memory condition ,compared to normal aging ,AD patients,s releasing of the 40HzERP reduced significantly. The brain area, each standard leads of TA, TT、D2 decreased obviously, Ck has no obvious change, and no encephalic region advantages and lateralization advantage. Conclusion:1.On non-linear dynamic parameters of 40HzERP,TA, TT, D2 have significant differences on quiet condition and memory condition, and could be used to describe 40HzERP characteristic parameters; Chinese characters and image memory events can be used as stimulation event to testi senior brain function.2.40HzERP of AD patients declined more obviously than normal people,and have no encephalic region differences and lateralization advantage, on 40HzERP non-linear dynamics parameters,TA, TT、D2 can be used to diagnose AD.
