【摘要】 乳腺增生病是女性最常见的乳房疾病,属中医“乳癖”范畴,以乳房疼痛及肿块为主要临床表现。其发病率占乳腺疾病的首位,本病在我国的发病率有逐年上升的趋势,年龄也越来越低龄化。由于对乳腺增生发生的机理和病因尚无确切了解,目前治疗上基本为对症治疗。近年来,随着针灸临床及机理研究的不断深入,针灸疗法在治疗乳腺增生病方面逐渐发挥重要的作用。而穴位埋线疗法是集多种方法、多种效应于一体的复合性治疗方法,具有其独特的临床实用价值。研究目的本课题通过观察穴位埋线法治疗乳腺增生病治疗前后的临床疗效,探讨疗效好、副作用小、有效提高生活质量的乳腺增生的针灸治疗方法。研究方法从广州中医药大学第一附属医院针灸门诊选取60例乳腺增生病患者,随机分为穴位埋线组30例和针刺组30例。治疗组和对照组选取相同的两组穴位：1.屋翳、期门、天宗、足三里；2.膻中、肩井、肝俞、丰隆(均双)。治疗组：每周治疗1次,两组穴位交替,一个月为一疗程,共治疗2个疗程。对照组：每周治疗3次,共治疗2个疗程。所有患者均于治疗前及治疗第一、第二疗程后,分别记录治疗前后总积分、伴随症状治疗前后积分等。建立数据库,运用SPSS17.0进行统计分析。研究结果经统计学分析,两组在年龄、病程、病情方面均具有可比性(P>0.05)；两组对乳腺疼痛、肿块及其他相兼症状的改善方面均有疗效,且第二疗程疗效优于第一疗程(P<0.05)；穴位埋线组和针刺组总有效率分别为96.67%和100%,经Ridit分析,两组治疗效果差异无统计学意义(P>0.05)。而其中对乳房肿块、情绪变化、月经异常、腰膝酸软、两胁胀满、瘀血等方面,两组均有疗效,且治疗效果相当(P>0.05)；但是在改善乳房疼痛方面,穴位埋线组疗效要优于针刺组(P<0.05)。结论临床穴位埋线治疗及针刺治疗对乳腺增生病均有较好的疗效,且穴位埋线对治疗乳腺疼痛方面疗效优于针刺组；即相比较普通针刺,埋线疗法能在较短的时间内、更大程度地减轻患者的疼痛。而且埋线疗法安全性高,发生不良反应少且轻微,易为患者接受,能减少患者的就诊次数,适应现代快节奏生活,更为经济实用,是一种安全、经济、行之有效的方法,值得临床推广应用。更多还原

【Abstract】 Background:Hyperplasia of Mammary Glands is an ordinary disease mostly among women, which has symptoms including pain or tumour in mammary glands. It has a first place attack rate in mammary disease; the rate is ascending year by year and the age of onset becomes younger and younger. Due to a deficiency of a certain mechanism or etiopathogenisis of Hyperplasia of Mammary Glands, it just has a symptomatic treatment to this disease at present. Recently following the progressing of the clinical and experiment of acupuncture, its effects on the Hyperplasia of Mammary Glands become bigger and bigger. And acupoint catgut embedding therapy is a complexity treatment of multi-therapy and multi-effect, it has its own distinct clinic practical value.Objective:To observe the curative effect of acupoint catgut embedding therapy in Hyperplasia of Mammary Glands before and after treatment, and to approach an effective acupuncture treatment which has a tiny side effect and can utility raise the quality of life.Methods:60 patients from the acupuncture outpatient department in the first affiliated hospital of Guangzhou University of Chinese Medicine were selected according to strict diagnosis criterion, divided to the treat group of 30 cases and the control group of 30 cases. Two groups select the same sets of acupoints:1.wuyi, qimen, tianzong, zusanli; 2. danzhong, jianjin, ganshu, fenglong(both sides). Treat group:treat once a week with two sets of acupoints alternated; put one month as a course of treatment, to cure two courses. Control group:treat 3 times/week(Monday, Wednesday, Friday), to cure two courses of treatment. We detected all patients’accumulated points and simultaneous phenomenon points before and after treatment. Record the data from the two groups and establish the database; analyze them by SPSS 17.0 software package.Results:By statistics analyzed, two groups are comparability from ages, courses of disease and pathogenetic condition (P>0.05). Two groups both have curative effect on mammary gland pain, tumor and other simultaneous phenomenon, and deuto-course of treatment is better than the prime course (P<0.05). The treat group total effective rate is 96.67%, while the control group reach to 100%, but the two groups have no statistical significance(P＞0.05). The curative effect between before and after treatment is significance, but the same between the two groups has no statistical significance (P>0.05). But on the mammary gland pain, treat group has a better curative effect than control group (P<0.05)Conclusions:Both two groups have a good curative effect to treat Hyperplasia of Mammary Glands, and acupoint catgut embedding therapy is better than acupuncture treatment on mammary gland pain. To compare normal acupuncture, acupoint catgut embedding therapy can cure the pain of mammary gland in a shorter time. And acupoint catgut embedding therapy is safe and has little adverse reaction, which is accepted by patients. With less times of treatment, fitting fast tempo of the modern time, it is economic and practical which is also safe, and is worth extending on the clinic.更多还原
