孤立性胎儿脉络丛囊肿的神经发育探讨

【摘要】 目的:探讨产前诊断孤立性胎儿脉络膜囊肿的儿童神经发育预后。方法:采用回顾性的队列研究,观察2008年1月至2009年6月在本院产前超声检查发现的胎儿脉络膜囊肿孕妇125例,以同期超声检查正常的孕妇119例对照,随访其妊娠结局和儿童神经发育情况。结果:125例胎儿脉络丛囊肿的儿童中,有30例(24.0%)存在神经发育不理想的情况,显著高于正常对照组9例(7.6%),相对危险度为3.86。但多数十分轻微。而脉络丛自身的特点,如囊肿大小,囊肿数目,囊肿位置以及囊肿吸收消失的孕周,均与儿童神经发育预后无明显相关。结论:排除染色体异常,孤立性胎儿脉络丛囊肿对儿童的神经系统发育十分轻微。

【Abstract】 Objective:The purpose of this study was to evaluate the neurodevelopment outcome of children who had choroid plexus cysts (CPCs) detected prenatally. Methods:Using a retrospective double cohort design, compare the development of children who had isolated CPCs detected prenatally with a control group of children who had normal prenatal ultrasounds during January 2008 to June 2009 by standardized neurodevelopment measures. Results:There were no differences on demographic variables between the CPCs cohort (n=125) and control cohort (n=119). Suboptimal neurodevelopmental outcome was found in 24.0% of CPCs cohort with is significant higher than control cohort, the risk ratio is 3.86.But most of their suboptimal neurodevelopmental outcome are very mild, which do not need treatment. While the charterers of CPCs, such as solitary or multiple, unilateral or bilateral, big or small and the time CPCs dissolve are not associate with the neurodevelopmental outcome.Conclusion:We conclude that the presence of isolated CPCs on prenatal ultrasound are not likely to be associated with dramatic neurocognitive delays in childhood, but we recommend to follow up the children regularly.
