【摘要】 目的:评价单眼视及Carl Zeiss Meditec MEL80 shotfile老视切削模式治疗伴有近视的老视患者的短期疗效。方法:对2008年1月-10月来我院眼科中心行LASIK的4例年龄43-50岁伴有近视的老视患者,主视眼接受标准的ASA模式矫正屈光不正,非主视眼用ASA模式预留一定近视球镜度数(-0.25～-0.50DS)后再用Carl Zeiss Meditec MEL80shotfile老视切削模式进行激光治疗,随访至术后3月,观察术后裸眼远、近视力,最佳矫正视力,等效屈光度,球差,对比敏感度,调节幅度。结果:4名患者术中未出现手术并发症,4名患者术后主视眼裸眼远视力均为1.0,非主视眼裸眼远视力均在0.8以上;非主视眼术后裸眼近视力较术前均有所提高;术后最佳矫正视力均无丢失;术后等效屈光度为预期值±0.50 D;主视眼术后球差较术前向负值方向漂移,而非主视眼术后球差较术前向正值方向漂移;主视眼及非主视眼眩光敏感度及对比敏感度术后一周有所下降,随着时间推移有恢复趋势,非主视眼在术后6.3-4.0deg的对比度阈值甚至高于术前;双眼调节幅度术后一周与术前比较有所下降,术后一月开始恢复,术后三月基本恢复术前水平。结论:单眼视准分子激光老视切削模式可在不影响获得较好的裸眼远视力的同时获得更好的裸眼近视力,手术安全有效。更多还原

【Abstract】 Objective To evaluate the short term clinical effect of LASIK for myopia and presbyopia using monovision and the Carl Zeiss Meditec MEL80 shotfile software.Methods Four myopic and presbyopic patients,43 years or older,who sought LASIK vision correction in the Eye Center of the Second Affiliated Hospital(Medical College, Zhejiang University) from January 2008 to October 2008 with a follow-up of 3 months were involved in this prospective study.The target refraction was plano for dominant eyes(distance eyes) and range -0.25 to -0.50 diopters(D) for non-dominant eyes(near eyes).Then the non-dominant eyes underwent the treatments of the Carl Zeiss Meditec MELS0 shotfile software.Distance and near uncorrected visual acuity,best spectacle-corrected visual acuity,spherical equivalent refraction,spherical aberration, contrast sensitivity,amplitude of accommodation preoperative and postoperative 1-week,1-month,3-months respectively were recorded.Results Operations were successful in all the cases without any complications.The distance uncorrected visual acuity(UCVA) was 1.0 in the dominant eyes and 0.8 or better in the non-dominant eyes after surgery,and the near vision of four patients improved in the non-dominant eyes.No eyes lost any line of best spectacle-corrected visual acuity(BSCVA).The postoperative spherical equivalent refraction was±0.50 D. The 4th spherical aberration decreased after the operation in the dominant eyes and increased in the non-dominant eyes(Malacara Notation).There was a general reduction in the contrast sensitivity in all spatial frequencies at postoperative 1-week when they were compared with the preoperative contrast sensitivity.By 1-month visit,all the eyes showed a recovery of the contrast sensitivity function with time going by and the contrast sensitivities at 6.3,4.0 deg spatial frequencies were better than that before the operation in the non-dominant eyes.So was the change of the binocular amplitude of accommodation.Conclusions LASIK for myopia and presbyopia using monovision and the Carl Zeiss Meditec MEL80 shotfile software is safe and effective.And it can provide better near vision by the maximal preservation of unaffected distance vision.更多还原
