【摘要】 目的:观察加味苏子降气汤对慢性阻塞性肺疾病急性加重期（以下简称AECOPD）患者的临床症状、体征、血气分析等的影响。方法:把60例慢性阻塞性肺疾病病情分级为Ⅰ～Ⅲ级的患者随机分为治疗组和对照组两组。治疗组用加味苏子降气汤+西医常规治疗,对照组用单纯西医常规治疗,疗程为10天。观察两组对COPD急性加重期患者临床症状、体征,血气分析指标等的影响。结果:1加味苏子降气汤能改善AECOPD患者临床证候本研究采用加味苏子降气汤治疗AECOPD患者,结果显示,两组治疗前后咳嗽、咯痰、喘息、纳呆、胸痛、哮鸣等各项主要症状及体征比较,差异具有显著性（p＜0.05或p＜0.01）,说明两组的治疗方案可明显改善患者的症状与体征。治疗组治疗后咳嗽、咯痰、喘息、纳呆、胸痛、哮鸣等各项主要症状及体征与对照组相比较,差异具有显著性（p＜0.05或p＜0.01）,提示治疗组的治疗方案在改善患者的症状与体征方面优于对照组。从证候积分综合评价,治疗组疗效优于对照组,具有显著性差异（p＜0.05）。2加味苏子降气汤能改善AECOPD患者缺氧和二氧化碳潴留研究结果显示两组治疗前后血气分析指标PO₂与PCO₂指标比较,具有显著性差异（p＜0.05）,提示两组治疗方案均可改善患者的缺氧及二氧化碳潴留;两组治疗后PO₂、PCO₂指标比较有显著性差异（p＜0.05）,提示治疗组改善患者缺氧及二氧化碳潴留的作用优于对照组。结论:采用加味苏子降气汤治疗AECOPD患者,能明显改善患者的临床症状和体征,取得良好的临床疗效;加味苏子降气汤能改善患者的缺氧及二氧化碳潴留。更多还原

【Abstract】 OBJECTIVEThe aim of the study was to assess the effect of Suzi Jiangqi decoction on AECOPD by analyzing the symptom,sign,blood gas analysis.METHOD60 AECOPD patients（stageⅠ-Ⅲ） with retention of Phlegm and Heat Syndrome in the Lung were randomly assigned to the treating group or to the control group.The treating group accept conventional therapy of the western medicine（Controllability oxygen treatment,bronchodilators,antibiotic,expectorants）plus Suzi Jiangqi decoction,while the control group purely accepted conventional therapy of the western medicine.The course was 10 days.The clinical symptoms and sign,blood gas analysis were observed and compared after 10 days.RESULTS1.The management of Suzi Jiangqi decoction could improve the clinical symptoms and sign of COPD patients.The clinical symptoms and sign of both group have significant difference after ten-day treatment compared with the clinical symptoms and sign before the treatment（p＜0.05或p＜0.01）.The clinical symptoms and sign of the treating group was better than the control group with significant difference（p＜0.05或p＜0.01） after the treatment,which showed the management of the treating group is better. 2.The management of Suzi Jiangqi decoction could remedy the state of anoxia and carbon dioxide retention.PaO₂ and PaCO₂ of each group have significant difference（p＜0.05） after the treatment compared with pre-treatment,that showed the managements of both groups could improve PaO₂ and reduce PaCO₂.After the treatment,PaO₂ and PaCO₂ of the treating group is better than the control group,with significant difference（p＜0.05）CONCLUSIONSThe management of the Suzi Jiangqi decoction can help AECOPD patients,it can improve the clinical symptoms and sign,remedy the state of anoxia and carbon dioxide retention.更多还原
