聚维酮碘氟化泡沫对龋易感儿童口腔致龋菌的抑制作用

Effectiveness of Povidine-iodine Fluoride Foam Treatment Against Cariogenic Bacteria in 6-9 Years Old Children in China

【摘要】 目的了解含有10%聚维酮碘的氟化泡沫对口腔致龋菌的抑制效果。方法将61名6-9岁龋易感儿童随机分为2组,实验组30人,对照组31人,分别使用含有10%聚维酮碘的氟化泡沫和常规氟化泡沫进行口腔局部处理,每周一次,连续处理四周。采用平板法进行唾液中细菌培养记数观测处理前,处理过程中每一周以及处理后1个月及4个月唾液中主要致龋菌(变形链球菌、乳酸杆菌以及可培养总菌落)水平的变化。结果每周一次连续四周使用含有10%聚维酮碘的氟化泡沫能较对照组更为有效降低唾液中变形链球菌水平,效果可持续到4个月后。然而,唾液中可培养总菌落和乳酸杆菌水平在泡沫处理前后各时间点组间和组内均不存在统计学差异。结论每周一次连续四周使用10%聚维酮碘氟化泡沫能明显降低唾液中变形链球菌水平,但不改变唾液中可培养总菌落和乳酸杆菌水平,其作用效果能持续到处理后4个月。相对于常规氟化泡沫,处理期内10%聚维酮碘氟化泡沫能更为明显抑制唾液中变形链球菌水平。提示持续反复使用10%的聚维酮碘可能有助于控制龋病易感儿童唾液中过高的变链菌水平。

【Abstract】 Objective: The objective of the present prospective study was to investigate the inhibitory effect of povidine-iodine/fluoride foam treatment on the levels of salivary Mutans streptococci (ms) and lactobacilli (lb). Method: Sixty-one 6-9 years old children with at least one active dental cavity were randomly assigned into two groups, and weekly treated with either povidine-iodine/fluoride foam (n=30, experiemental group) or placebo fluoride foam(n=31,control group) over 4 weeks. Stimulated saliva samples were collected each time prior to the treatment and 1 month as well as 4 months after the final foam treatment. The samples were cultured and colony forming units were enumerated. Results: A constant decrease of salivary ms was observed during the foam treatment and a plateau effect was achieved over the following 4 months comparing with the baseline in both the two groups. Moreover, the povidine-iodine/fluoride foam resulted in a higher decrease of salivary ms levels during the period of 4 successive foam treatments compared with the placebo fluoride foam. However, total cultivable facultative bacteria and lb levels in saliva remained stable during the treatment for both the two groups and no statistically significant differences were investigated. Conclusion: The results indicate that weekly application of povidine iodine fluoride foam over four weeks could lead to a significant suppression on salivary ms levels for four months without affecting the total cultivable facultative flora. And the repeated application of povidine iodine fluoride foam might lead to a long term suppression of salivary ms levels among the high-caries-risk children.
