两种粘结剂对托槽周围釉质脱矿程度及氟水平的影响

Effect of Two Kinds Enamel Adhesives on Demineralization Around Orthodontic Bonding Agents and Fluoride Level

【摘要】 目的: 比较含氟牙膏对两种釉质粘结剂(含氟和不含氟粘结剂)在托槽周围釉质脱矿程度以及氟水平的影响。 方法: 第一部分,体外实验。将40个离体体牙分四组,每组10个。第一组和第二组用京津釉质粘结剂(composite resin)粘结,第三组和第四组用自酸蚀粘结剂(ESPE,fluoridated adhesive)粘结,模拟口腔环境,进行体外PH循环,循环周期为30天。第一组和第三组每天浸泡含氟牙膏三分钟,第二组和第四组不浸泡含氟牙膏。收集每天的酸蚀液和人工唾液各1ml。30天后同一测试者用氟离子选择电极检测每天总的氟水平;体视显微镜评估釉质脱矿指数,一周后同一测试者进行重复测定。用SPSS11.5软件包对氟水平和龋坏指数进行单因素方差分析,并对两次测量的龋坏指数进行Kappa一致性评价。 第二部分,体内实验。选择5个需正畸拔牙的患者,每人的左上和右下前磨牙用自酸蚀粘结剂粘结,左下和右上前磨牙用京津釉质粘结剂粘结,并嘱其每天使用同一品牌的含氟牙膏3次。一个月后拔除,冠根分离,用显微硬度仪进行显微硬度测量,用显微硬度值来评价其釉质脱矿的程度(两者的相关系数为0.91)。用SPSS11.5软件包对两组显微硬度值进行配对t检验,并对25微米深度处的硬度值进行析因分析。 结果: 第一部分,单因素方差分析得出除了第一组和第四组之间,其它各组之间的氟水平有显著性差异(p<0.01)。其中第三组氟水平最高;第一组和第四组氟水平次之并且相当;第二组最低,含氟量可以忽略。龋坏指数与氟水平呈负相关性,四组之间均有显著性差异。第二组最大,第四组次之,第一组再次之,第三组最

【Abstract】 Objective:To compare the effect of two kinds enamel adhesives on enamel demineralization around brackets and the fluoride level, and compare the effect of extrinsic fluoridated dentifrice on the demineralization of each material.Methods:1. In vitro, extracted premolars were divided into four groups. Orthodontic brackets were bonded with enamel adhesives. Teeth of group 1 and group 2 were bonded with Jingjin enamel adhesive, group 3 and group 4 with ESPE. The teeth were subjected to PH cycling, over a 30-day period. Teeth of group 1 and group 3 were immersed in fluoridated dentifrice for 3 minutes each day. Fluoride release was measured using fluoride ion-selective electrode. After 30 days they were assessed visually for enamel caries index. After a week carried index were assessed again to evaluate the consistence using Kappa analysis. We use the SPSS 11.5 soft kit to make one-way analysis of variance (ANOVA) to analyze the fluoride level and carried index.2. In vivo, 5 orthodontic patients were scheduled to have premolars extracted. Maxillary left premolar and mandibular right premolar were bonded with ESPE, and the others two were bonded with Jingjin enamel adhesives. They were told to use fluoridated dentifrices of the same trademark 3 times each day. After 30 days, the teeth were extracted and longitudinally sectioned to measure cross-sectional microhardness to assess the extend of enamel demineralization.
