过敏性猝死豚鼠IgE含量的死后变化规律及肥大细胞脱颗粒的观察

【摘要】 目的:研究过敏性猝死豚鼠死后在4℃冷藏条件下血清IgE含量随死后时间变化的规律,观察过敏性猝死豚鼠肺组织内IgE的免疫表达随死后时间变化的规律,同时用组合染色方法显示肥大细胞脱颗粒现象,并探讨其在法医学鉴定中的作用和意义,试图为过敏性猝死的法医学鉴定提供准确、客观的指标。 方法:豚鼠64只,雌雄不限,体重200g左右,随机分为实验组和对照组,对照组和实验组又分别分为死后0h组、死后12h组、死后24h组、死后48h组,每组8只豚鼠。实验组采用多人混合血清注射液致敏豚鼠,建立过敏性休克死亡的动物模型,取豚鼠血清及肺组织。(1)用酶联免疫吸附实验检测豚鼠血清IgE含量。(2)采用免疫组化ABC法对过敏性猝死豚鼠肺组织IgE进行免疫组化染色,用BI2000病理图像分析系统对组织切片进行分析。(3)应用俾士麦棕—碘绿—马汀黄(BB-IG-MY)组合染色法显示肥大细胞脱颗粒现象。(4)数据的处理,采用SPSS 11.5统计软件,两因素方差分析。 结果:(1)实验组豚鼠在被抗原再次攻击后20s左右出现活动减少,1min左右出现休克症状,2-5min死亡,死亡率100%。对照组豚鼠均未出现任何症状。(2)过敏性猝死豚鼠与对照组豚鼠血清IgE含量相比较,p<0.001,有统计学意义,可认为过敏性猝死豚鼠血清IgE含量显著升高;过敏性猝死豚鼠死后0h组、死后12h组、死后24h组、死后48h组血清IgE含量相比较,p>0.05,无统计学意义,尚不能认为过敏性猝死豚鼠死后在4℃冷藏条件下血清IgE含量在48小时内随时间延长有明显的变化。(3)免疫组化染色结果表明,过敏性猝死豚鼠肺小血管壁及血管周围均出现浅黄色或棕黄色颗粒,对照组豚鼠上述组织部位无浅黄色或棕黄色颗粒。过敏性猝死豚鼠死后0h组、死后12h组、死后24h组、死后48h组肺组织IgE平均分布密度相比较p>0.05,说明过敏性猝死豚鼠在4℃冷藏条件下肺组织IgE含量在死后48h内无明显变化。(4)组合染色法结果表明,肥大细胞胞质颗粒呈棕色,细胞核成绿色,红细胞呈黄色。过敏性猝死豚鼠肥大细胞脱颗粒现象明显,而对照组豚鼠肥大细胞未见脱颗粒现象。 结论:过敏性猝死豚鼠血清IgE含量显著升高。过敏性猝死豚鼠死后在4℃下冷藏,48h内血清IgE含量和肺组织中IgE随死后时间延长无明显变化,因此应用酶联免疫吸附实验检测血清IgE含量,或对肺组织进行IgE免疫组化染色,可为过敏性猝死的法医学鉴定提供客观的诊断依据。过敏性猝死时,豚鼠肥大细胞有明显的脱颗粒现象。

【Abstract】 Objective Research the change regularity of serum IgE in the guiea pig death of anaphylactic shock that is cold-storaged in 4°C,and study mast cell degranulation with combined- staining,and investigate the role and meaning in the medicolegal expertise. So we can provide accurate and objective diadynamic criteria to the sudden erethistic death for the medicolegal expertise.Methods 64 guinea pigs are classified into the experimental group and the control group, with no purpose and each groups are classified into 0 hour after death group、 12 hour after death group、 24 hour after death group、 48 f hour after death group(Every group contains 8 guiea pigs).Establishing guinea pigs anaphylactic shock model,and making specimens of blood serum,and lung. (1)we tested the serum levels of IgE with ELISA. (2)we observe IgE in the lung of the guinea pig death of the anaphylactic shock with immunohistochemistry. (3)we study mast cell degranulation with BB-IG-MY combined-staining .Results (1)The concentrations of IgE of experimental group are higher than that of the control group.The serum level of IgE is no difference in the experimental groups 48 hours after death. (2)The result of immunohistochemistry dyeing indicate that there is some some pale yellow granula in the lung small vessle wall of the experimental groups.(3)Mast cell have released their granules.Conclusion Measuring the serum level of IgE can provide objective diadynamic criteria to diagnose anaphylactic shock for forensic experts. Mast cell have released their granules in the guiea pig death of anaphylactic shock.
