年轻成人上颌快速扩弓(YA-RME)对牙颌结构的影响

【摘要】 [目的] 本研究采用先进的三维激光扫描测量技术,并结合传统的模型测量与X线头影测量技术,以替牙期后牙反(牙合)患者为对照组,系统化分析评价年轻成人上颌快速扩弓(YA-RME)后患者牙颌组织结构的三维立体变化,研究内容包括扩弓对患者腭中缝、上颌牙弓、上颌牙槽骨、腭部表面形态、临床牙冠高度以及下颌平面角和下面高等方面的影响,为临床上更好地使用年轻成人上颌快速扩弓技术(YA-RME)提供理论依据与技术支持。 [材料与方法] 按照标准选择在山东大学口腔医院正畸科就诊的13例年轻成人后牙反(牙合)患者为观察组,其中男5例、女8例,年龄16.6-19.2岁,平均17.8岁。同时选择20例替牙期上颌快速扩弓患者作为对照组,其中男8例,女12例,年龄7.4-11.8岁,平均9.5岁。所有患者均使用塑料基托粘结式上颌快速扩弓装置进行扩弓矫治,扩弓前(T1期)扩弓后(T2期)分别制取患者牙颌硬石膏模型,并拍摄X线头颅侧位片和上颌咬合片。牙颌模型测量指标18项,X线头影测量指标2项;采用三维激光扫描技术获得腭部表面形态的数字化模型,截取6-6腭部表面形态轮廓线,采用Handelman分析法进行测量分析,测量指标5项。所有测量数据采用SPSS11.0统计软件对测量数据进行分析,t检验,α为0.05。 [结果] 1.年轻成人上颌快速扩弓后,3-3牙弓宽度增加2.8±2.1mm,4-4牙弓宽度增加5.2±2.5mm,5-5牙弓宽度增加5.7+2.9mm,6-6牙弓宽度增加4.9±3.0mm。与儿童扩弓患者相比,各项指标均不具有显著性差异。

【Abstract】 [Objective] The purpose of this study was to evaluate the dental and skeletal changes of posterior crossbite young adults after rapid maxillary expansion with the use of 3-dimension laser scanning technology, traditional dental cast and cephalometric measurements, which including the changes of mid-palatal suture, maxillary dental arch, alveolar arch, palatal contour, clinical crown height, mandibular plane angle, and the lower facial height.[Methods] The observation group included 13 YA-RME subjects, while the control group included 20 RME subjects in mixted dentine. All of them were treated with acrylic bonded maxillary expansion appliance, and records were taken at the pre-treatment time (T1) and post-treatment(T2) including maxillary transverse arch width, gingival width, alveolar width, arch length, arch perimeter, clinical crown height, molar angle, palatal vault angle, palatal width at gingival height, palatal width at mid-palate, palatal width difference, MP-SN angle and ANS-Me distance, of which the measurements of palatal contour were derived by the use of 3-D LST. Students’ t-test were used to determine if there were significant differences between 2 time periods and between 2 groups with SPSS 11.0 software.
