肥胖青少年认知功能心理学评定与额叶、海马代谢的磁共振波谱研究

Psychological Evaluation of Cognitive Function and ~1H-MRS Study of Frontal Lobe and Hippocampus Metabolism among Obese Adolescents

【摘要】 目的 研究肥胖青少年推理能力以及智力水平;比较肥胖组和对照组青少年在一定脑力负荷后,注意能力是否存在差异。同时采用氢质子磁共振波谱(proton magnetic resonance spectroscopy,~1H-MRS)技术,初步探讨肥胖青少年额叶和海马感兴趣区脑代谢的特征。 方法 对选取的250名来自安徽省两座城市中学初一、初二、高一、高二的学生进行体成分测定,以BMI作为划分肥胖的标准,共选取肥胖及对照青少年82对。使用韦氏儿童智力量表中国修订本(Chinese Wechsler Intelligence Scale for Children,C-WISC)(小于16岁)或韦氏成人智力量表中国修订本(Wechsler Adult Intelligence Scale Chinese Version,WAIS-CR)(17岁以上)中的数字广度测验、木块图测验、算术测验、填图测验以及瑞文标准推理测验。以瑞文标准推理测试、韦氏智力量表中部分项目测试60~70min作为脑力作业负荷,负荷前、后分别进行划消试验和视觉追踪测验,统计划消试验的阅读速度、错误率、脑力工作能力指数(index of mental capacity,IMC),统计视觉追踪试验的完成时间及正确数,分析脑力作业负荷对肥胖青少年推理能力、智力及注意力等认知功能的影响及两组间的差异,并分析肥胖青少年注意力的特点。分别对8名中重度肥胖青少年及配对的正常体重青少年进行人体测量(身高、体重、腰围、臀围)和瑞文推理测验,使用美国GE公司生产的LUNAR双能X线吸收仪(dual energy X-ray absorptiometry,DXA)全身扫描,测定全身以体脂百分比(fat mass percentage,FM%)。按照FM%判定中中重度肥胖的标准,采用氢质子磁共振波谱(~1H-MRS)检测额叶、海马部位的脑代谢特点。 结果 肥胖组推理能力的五个因子得分、推理智商(intelligence quotient,IQ)总分均低于正常对照组,但差异无显著性(P>0.05)。数字广度、算术、填图、木块图方面,肥胖组得分低于对照组,但差异无显著性。脑力作业负荷后,肥胖组划消试验阅读速

【Abstract】 Objectives The study aims to explore reasoning and intellective capacity in obeseadolescents and to provide scientific basis for the prevention and treatment on child obesity. And determine variation characteristics of attention ability in obese adolescents by giving mental workload. To find whether obese adolescents had neuro-chemical abnormalities in frontal lobe and hippocampus.Methods The intellectual and deduction characteristics of 82 obese adolescents andadolescents, aged 12 to 18 years, were compared to those of 82 normal-weight adolescents and adolescents of the same age group, utilizing Raven’s Standard Progressive Matrices ( R’ SPM) and four subtests of Wechsler Intelligence Scale for Adolescents-Chinese revision (C-WISC). Adolescents of the two groups were paired according to age groups, height, schooling level and parental educational level. Average scores of all indices were calculated and compared between the two groups. Mental workload was manipulated by presenting subjects with five tests, including Raven’s Standard Progressive Matrices (R’ SPM) and four subtests of Wechsler Intelligence Scale for Children-Chinese revision (C-WISC) in 60 to 70 minutes. Reading velocity, mistake rate, index of mental capacity (IMC) in Number Cancellation Test and test time, the right number of tracking lines in Visual Tracking Test were calculated and compared between the two groups. Anthropometric indicators were measured in all participants. Intelligence quotients (IQs) were assessed by using Raven’s Standard Progressive Matrices (R’ SPM). Proton magnetic resonance scans (~1H-MRS) were performed in 8 obese adolescents and 8 non - obese adolescents as control. Spectral peaks of N-acetylaspartate (NAA), Creatine (Cr), Choline (Cho) of two cortexes were measured and their ratios were computed and compared.
