双波长脑血氧检测的信号处理及其临床应用
Signals Processing and Clinic Application on Detecting the Parameters of Brain Blood and Oxygen with Dual Wavelengths

【摘要】 本文作者在在研究、解剖了武汉大学一海生物医学工程的“近红外双波长脑血氧监测仪”的基础上,编写了脑血、氧波形功率谱和相关函数的分析软件并分别在荆州中心医院和武汉大学采集72例人脑组织血、氧信号,应用该软件对采集的脑血、氧数据进行波形分析和临床研究。研究结果表明 1) “近红外双波长脑血氧监测仪”检测的正常人及麻醉后病人脑血氧饱和度与值为80%左右,符合正常生理参数,心功能不全患者脑血氧饱和度平均值为72.8%,明显低于正常人及麻醉后病人,医学统计学分析P<0.05,呈显著性差异。 2) 正常人脑血、氧波形信号显示血、氧波形中波峰、波谷对应良好;波形频率和节律稳定,心功能不全患者,脑血、氧波形信号表现为波动的频率和节律均不规则,其血、氧波动信号之间的对应性差;提示近红外光在生物组织中传递时可带来较为直观的脑组织血、氧变化信息; 3) 脑缺氧病人的功率谱曲线主频峰不清晰、功率谱基底宽,有许多活跃的频峰,血、氧的功率谱曲线吻合性差,与正常人脑血、氧波形主频峰清晰、功率谱基底部绝少主频峰外的其它频峰功率谱图存在明显差异; 以上结果证明:“近红外双波长脑血氧监测仪”该仪器可无创、准确监测脑血氧,其检测的脑血氧波形、功率谱、互相关系数作为脑血氧的辅助诊断参数。
【Abstract】 Based on research and anatomization of the "near infrared analytical instrument to blood and oxygen of brain" made by Center of Biomedical Engineering Wuhan University-Wuhan E-Sea Digital Techniques Co.,Ltd, the author writes the analytical software of wave about blood and oxygen of brain, power wave and correlation function and applied the software to wave analysis and clinic research on data about blood and oxygen gathered from 72 persons in center hospital of JingZhou. The research result shows:1) Saturation of brain examined from health person and anaesthesia person with near infrared analytical instrument to blood and oxygen of brain is about 80 percent, which accords with the normal physiological parameter, but average value about blood and oxygen of saturation brain checked from person who heart function not full is 72.8 percent and it is below the value from health person and anaesthesia person and the medical statistical analyses P less than 0.05,which shows the marked difference.2)The signal of wave about health person brain blood and oxygen demonstrates the wave crest of wave about blood and oxygen corresponds well with the trough and the frequency of the wave and rhythm stabilize, but as for the heart function not full, the signal of wave about brain blood and oxygen behaves the frequency and rhythm irregular, the signal between blood fluctuate and oxygen fluctuate don’t correspond well. This is a hint that near infrared passing through tissue can bring the intuitionistic and changing information of brain blood and oxygen.3) Power wave main frequency apex of person who brain lack of oxygen is unclear, and power wave floor is wide, and there are many active frequency apex, and power wave of blood and oxygen don’t tally well, while frequency apex of health person brain blood and oxygen is clear and there is a little main frequency apex and there exists the distinct difference.The results above prove that "near infrared analytical instrument to blood and oxygen of brain" may supervise the brain blood and oxygen harmlessly and correctly, and wave of brain blood and oxygen, power wave, correlation function examined by the instrument can become the parameters during the diagnosis on brain blood and oxygen.
