强脾补精化瘀益智法治疗老年性痴呆的相关研究

【摘要】 本文收集了大量古今中外文献资料,综合各家所论,从理论和实验两个方面,多角度、多层次地探讨了以强脾为主论治老年性痴呆(AD)的相关研究,为临床寻求治疗AD的有效方药提供可能的理论和实验依据。 在理论研究部分,从脾胃在机体衰老过程中的重要作用入手,我们认为AD是病理性衰老产生的疾病之一,是衰老的脑病态表现。根据中医“藏情相关”理论,从“脾藏营,营舍意”论点出发,运用中医脾胃理论及脾与脑的相关性,深入分析探讨了“脾胃虚衰”在AD发病中具有主导作用,为此确立了“强脾补精化瘀益智法”作为治疗AD的基本法则,并研制出“强脾补精化瘀益智胶囊”作为治疗AD的用药。在以往临床和实验取得一定成果的基础上,我们又设立了动物实验以进一步揭示其作用机制。 在动物实验部分,我们通过塑造AD动物模型,设立正常对照组、模型组、脑复康组(西药组)和强脾补精化瘀益智胶囊组(中药组),观察强脾补精化瘀益智胶囊对AD模型大鼠空间学习记忆能力的影响,检测AD大鼠脑海马匀浆液和血清乙酰胆碱酯酶(TChE)活性和B型单胺氧化酶(MAO-B)活性水平,以及测定血清中IL-1β、IL-6的含量并与其它组别进行比较,以期揭示强脾补精化瘀益智胶囊对AD的治疗作用机理。结果,中药组AD大鼠经过治疗,其空间学习记忆能力明显改善,与模型组比较有显著性差异(P<0.05或P<0.01);中药组脑海马匀浆组织及血清中TChE活性明显升高,MAO-B活性水平明显降低,与模型组比较有非常显著性差异(P<0.01);中药组AD大鼠血清中IL-1β和IL-6含量明显降低,与模型组比较有显著性差异(P<0.05,P<0.01)。提示强脾补精化瘀益智胶囊可能通过改善中枢胆碱能神经系统和单胺能神经系统,保护海马神经元,并抑制与老年斑形成有关的炎症反应,调整机体的免疫状态等多个环节从而达到改善学习记记,治疗AD的作用。

【Abstract】 This thesis collected a lot of old and new, domestic and foreign data and information and synthesized various theories. In order to provide the possible theoretical and experimental evidences for effective prescription which is suitable to treat AD in TCM clinical practice, and also discussed the relative researches of treating Alzheimer’s Disease (AD) mainly by invigorating spleen from various theoretical and experimental levels.In the theoretical study section, this thesis considered that the asthenia of spleen and stomach dominates pathological dotage, and AD is the appearance of brain aging. Then according to the theory of the relationship between Zang Organ & emotion, beginning with the point of "spleen storing nutritional essence and nutrient QI(the spleen) housing idea", and using the Spleen-stomach Theory of TCM and the relationship between spleen and brain, this paper probed thoroughly into what important effects spleen and stomach have in the mobility process of AD. We established the basic constitution of treatment to AD, which was invigorating the spleen and reinforcing the essence and removing blood stasis to refresh mental activities and developed the QIANGPI BUJING HUAYU YIZHI Capsule (QPC) to treat AD. Basing on the past effective results of clinical and animal studies, we designed the animal experiment to take further step to reveal the possible mechanism how the method of the treatment acted.In the experimental study section, the rats were divided into four groups, including the normal control, the model, the Piracetam (the western medicine), the QPC (the TCM), and copied to AD model. After the treatment, we observed the effects on the AD rats’ ability of leaning and memory, detecting the TChE activity and MAO-B activity of the AD rats’ brain hippocampus homogenate, and the TChE activity and MAO-B activity, IL-1 β content, IL-6content of AD rats’ blood serum, then comparing the QPC with other groups to reveal the mechanism of action of the QPC treating AD to bright. The results showed that, after treatment the group of TCM rats had been obviously improved the ability of learning and memory in space, with marked differences comparing to the model group(P<0.05. P< 0.01); and had been obviously increased TChE activity and lowed MAO-B activity of brain hippocampus homogenate with very marked difference compared to the model group (P< 0.01) ,and having low levels of IL-1 β content and IL-6 content of blood serum with a marked difference from the model group (P<0.05, P<0.01) . It indicated that QPC could improve the learning and memory ability to treat AD probably through better cholinergic nerve center system and monamine nerve center system and protecting hippocampus neuron, restraining the inflammation reactions which may have relations with the formation of senile plague.
