王好古《医垒元戎》学术思想研究

【摘要】 王好古,字进之,晚号海藏老人。元代赵州(今河北赵县)人,执业于晋州(今山西太原)。家世及具体生卒年不详(一说约于公元1200-1308 年),但传早年以通经举进士,官赵州教授,兼提举管内医学。他二十多岁时受业于易州名医张元素,与李东垣同窗,但少于东垣二十岁。元素殁,又从师于李杲,尽得其传,是易水学派的中坚人物。王好古毕生孜孜不倦,潜心研究医理,著述甚丰,现存有《医垒元戎》、《阴证略例》、《此事难知》、《汤液本草》和《斑论萃英》等,皆为历史上重要医学文献。此外,王氏尚有《仲景详辨》、《活人节要歌诀》、《三备集》、《光明论》、《小儿吊论》、《伤寒辨惑论》、《辨守真论》、《十二经药图解》、《仲景一集》、《汤液大法》、《钱氏补佚》等二十余部著述见于记载。《医垒元戎》是王好古的代表作之一,是伤寒外感与内伤杂病合一而论的专著。全书以六经分证,采用“先足经从汤液,后手经从杂例”的体例,即每一经首列伤寒证治,次列内伤杂病、补列杂病诸方,理论以张仲景为宗,参以易水之法,同时更有作者的心得发明。总之,将伤寒与杂病皆归于六经分证的内外一统论是《医垒元戎》的创举。该书内容丰富、议论精当、结构分明、重点突出,有着较高的学术价值和临床实用价值。书中大量引用历代特别是宋金元时期的诸多名家方药,以及易水门方和王氏自创方,而主治病证囊

【Abstract】 Wang Haogu (about 1200 BC—1308 ),born in zhaozhou (now Zhao County of Hebei Province),engaged in medical profession in Jinzhou (now Taiyuan of Shanxi Province). He was ever appointed as an official in charge of the medical affairs in Zhaozhou. In his twenties, Wang learned from the famous doctor Zhang Yuansu in Yizhou, and Li Dongyuan was his classmate, but 20 years senior of Wang. After the death oZhang, Wang learned medical skill from Li. Therefore, Wang Haogu mastered the essence on how to use traditional Chinese medicine and became one of the pillar of YiShui School. Wang Haogu devoted his heart and soul to traditional Chinese medicine, as a result, he wrote many works, including YiLeiYuanRong, Brief Elaboration of Yin Syndrome, Decoction of Material Medica and so on . YiLeiYuanRong is one of Wang Haogu’s representative works and is the monograph on the theory to unify exogenous and endogenous diseases by syndrome-differentiation of the six meridians. This book taught to diagnose and treat disease whether exogenous or endogenous based on syndrome different-iation of the six meridians. As to every meridian, firstly to discuss exogenous disease. Subsequently to analyze endogenous disease and give the proper prescription. This kind of theory originated mainly from the academic thoughts of Zhang Zhongjing and YiShui School. In a word, it was a pioneering work to create the theory on unifying exogenous and endogenous diseases by syndrome-differentiation of the six meridians. This book was briefly characterized as abundant contents, exact arguments, concise configuration and explicit emphasis, thus provided higher academic and clinic merits. The writer cited numerous theories and experiences from many celebrated medical sages from different periods, especially Jin-Yuan Dynasty, which made this book have a great value as ancient medical literature. YiLeiYuanRong became the book at first in 1291, but the original manuscript threw to lose afterward, and was recalled by author, completed most parts of the contents and republished it in 1297. YiLeiYuanRong existing various edition, was induced to have two types, twelve-volume and one-volume. According to research, the twelve-volume YiLeiYuanRong was written by Wang Haogu originally. The one-volume was just the extracts not the whole the book, which was included at the beginning in JiShengBaCui. By the full-scale and systematic research and summary to this book, the academic thoughts on this book were mainly embodied in the following aspects: First, to advocate the theory on unifying exogenous andendogenous diseases by syndrome-differentiation of the six meridians. He thought that exogenous and endogenous diseases affected mutually and contacted closely,and the two kinds of diseases could not be separated strongly,but they were both able to result in the insufficiency of vital qi. Therefore, Wang Publicized the theory to unify exogenous and endogenous diseases. This kind of standpoint was with science and rationality, and was the core of the academic thoughts about YiLeiYuanRong Second, to inherit and develop the medical thoughts of YiSui School. Wang was one of the pillar of YiShui School, he emphasized the syndrome-differentiation of the zang-fu organs and the importance of the function on spleen and stomach to human body, and inherited the theory of the target of the medication, and so on, which were embodied in YiLeiYuanRong. Therefore, Wang inherited and developed the academic thoughts of YiShui School completely. It is well-deserved to call Wang Haogu as one of the pillar of YiShui School. Third, Putting forward the viewpoint of syndrome differentiation based on the triple energizer and qi and blood. This view originated from the pattern on syndrome differentiation of the zang-fu organs and medication of zhangyuansu and was separated from the pattern by wang Haogu, Which was developed by the school studying seasonalfebrile disease.
