视网膜神经纤维层检查在青光眼早期诊断中的应用分析

【摘要】 目的 应用无赤光眼底照相及图像处理技术观察视网膜神经纤维层形态,并与视野检查结果进行对比分析,探讨视网膜神经纤维层检查在青光眼早期诊断中的临床意义。方法 选30例(55眼)门诊可疑的青光眼病人,进行无赤光眼底照相及视野检查,经Photoshop图像处理软件对眼底图像进行处理后观察,并与视野检查结果进行统计学检验及对比分析。结果 以无赤光眼底照相及图像处理技术观察视网膜神经纤维层形态方法的敏感性为94.60%,阳性率为78.19%,视野检查的阳性率为67.27%,经卡方检验,差异有统计学意义(X~2=15.0349,P=0.0001),提示RNFL观察的阳性率高于视野检查,结合临床,认为RNFL观察法实际上是较视野检查更早的发现了神经纤维的损害。结论应用无赤光眼底照相及图像处理技术观察视网膜神经纤维层形态,灵敏度较高,能够较视野检查更早的发现神经纤维的损害,为青光眼早期诊断、随访监测及治疗方案提供参考,且方法简单易行,适于普及,值得在青光眼早期诊断中推广应用

【Abstract】 objective To discuss the clinic significance of the examination of the retina nerve fiber layer in the diagnosis of the glaucoma, we observed the image of the retina nerve fiber layer with the red-free fundu photographs and image manipulation, and compared and analyzed it with the visual field. Methods A red-free fundu photograph and a visual field examination were taken for 30 out-patients (50 eyes) who were suspicious of the glaucoma, the fundu photographs were treated with Photoshop software, and were statistical treatmented and analyzed with the visual field. Results The sensitivity and the abnormal rate of the methods of the the red-free fundu photography and image manipulation is 94. 60%, 78.19%, and, is 81. 40% and 67. 27% in the visual field examination respectively, the difference has statistical significance by X2 test, so, we consider that the methods of the red-free fundu photography is earlier than the visual field examination in detecting the damage of the retina nerve fiber layer. Conclusion the methods of the red-free fundu photography and image manipulation is worthy of popularizing, because it can detect the damage of the retina nerve fiber layer earlier than examination, which affords reference for the early diagnosis and therapy of the glaucoma, further more, it is easy to apply and operate in everywhere.
