安肾胶囊治疗难治性肾病综合征痰瘀互结证的实验研究

【摘要】 目的:安肾胶囊(ASC),是临床治疗难治性肾病综合征(RNS)的有效验方,具有消痰软坚散结之功。本课题以中医药理论为指导,采用现代医学技术手段,观察安肾胶囊对模型大鼠的治疗作用,评价和探索治疗难治性肾病综合征的疗效和作用机理,为临床用药提供依据。 方法:实验采用阳离子化牛血清白蛋白(C—BSA)多次尾静脉注射方法复制大鼠膜性肾病(MN)模型,随机分为正常对照组、模型对照组、雷公藤多甙组、安肾高、中、低剂量组。4周造模完成后,进行灌胃治疗4周,定期观察大鼠一般情况、测定治疗前后24小时尿蛋白定量、血尿程度、总蛋白、白蛋白、甘油三脂、胆固醇,并通过光镜体视学定量测定和电镜观察肾脏病理改变。 结果:(1)安肾胶囊能降低MN大鼠24小时尿白蛋定量,用药各组与模型组比较有显著性差异,尤以中剂量组效果明显(P<0.01)。(2)可以减轻MN大鼠血尿程度,用药各组与模型对照组比较有显著性差异(P<0.05)。(3)能升高模型大鼠白蛋白,降低甘油三脂和胆固醇。(4)病理形态观察,安肾胶囊能减轻模型大鼠肾脏组织病变,减轻肾小球肿大、基底膜增厚,改善足突融合,尤以减少分叶肾小球数目明显(P<0.05)。 结论:安肾胶囊对大鼠膜性肾病具有降低蛋白尿、血尿、改善低蛋白血症、调节脂质代谢紊乱和修复肾小球病理损伤的作用,作用效果与雷公藤多甙片相当。其作用机制可能与中药复方通过多途径、多环节恢复肾小球滤过屏障功能有关。
【Abstract】 Objective: An shen capsule (ASC) is based on long-term and good effect for refractory nephropathy syndrome (RNS), which is an experience compound prescri--ption. It has the action of eliminating phlegm, softening hard masses and removing blood stasis. To observe the curative effect of ASC on glomerulonephritis rats, asse--ss and explore its therapeutic efficacy and potential mechanism for RNS.Methods: Membranous nephropathy (MN) rats were produced by cationized bovine serum albumin (C-BSA). After one-week preimmunization, C-BSA had been injected in rat’s tail vein one time every other day for 3 weeks. Except the nor--mal group, the animals were divided into five groups randomly, including the mod--eled group, the high-dose, the middle-dose, the low-dose, the tripteryguim wilfordii polyglycosidium (TWP) group. They were measured the proteinuria of 24 hours, hematuria, total protein, albumin, cholesterol, triglyceride and renal pathology rese--arch.Results: (1) ASC can reduce proteinuria and hematuria of MN rats. There are differences among the groups (P<0.05). Especially, the middle-dose group showed significantly (P<0.01). (2) ASC could in- increase albumin and decrease cholesterol and triglyceride. (3) It showed that ASC and TWP could relieve pathology changes of kidney, which are glomerulus’s tumefaction, basement membrane’s thickening, foot process’s confluence and glomerulus’s sclerosis.Conclusion: ASC has effect on RNS as good as TWP. It exerts the effect poss-ibly by regulating abnormal situation of glomerulus’s filter membrane in MN rats.
