情志应激诱发实验性2型糖尿病及中药干预的实验研究

【摘要】 一、目的 本课题从糖尿病多因素致病角度出发,探讨情志因素对糖尿病发病的作用机理,以及具有理气降逆散结功效的复方中药心身5号方的调节作用。 二、方法 本实验依据中医情志致病理论,结合现代生物医学和心身医学的观点,采用腹腔注射少量链脲佐菌素(STZ)联合旋转、拥挤、限制三种心理生理实验方法制作了应激诱发2型糖尿病病证结合的大鼠模型。实验动物共分正常组、应激组、STZ组、应激+STZ组和中药组五组。观察大鼠一般状态的改变并检测了各组大鼠血糖、血清胰岛素、血清胰高血糖素、肝脏cAMP含量和胰岛素基因mRNA的表达量,同时观察了各组大鼠胰腺组织形态变化和宏观行为改变。 三、结果 1.情志应激可促使少量腹腔注射STZ大鼠血糖显著持续升高,血中胰岛素浓度降低,与单纯应激组单纯STZ组大鼠比较,有显著性差异(p<0.01或p<0.05),同时伴有大鼠的宏观行为学发生改变,旷场实验中,中央格时间、穿行格数、直立次数与单纯应激组、STZ组相比有显著性差异或极显著性差异(p<0.01或p<0.05),以及一般状态与单纯应激组、STZ组相比有差别,提示该模型符合2型糖尿病气机失调病证特征。中药组血糖明显降低,血清胰岛素含量提高,宏观行为状况较模型组有改善。 2.情志应激可促使STZ鼠血清胰高血糖素含量,肝脏中cAMP明显升高,与其它组比较有极显著性差异(p<0.01),中药可降低血清胰高血糖素含量,降低肝脏中cAMP含量。 3.情志应激可降低大鼠胰岛素mRNA的表达量。STZ+应激组大鼠胰岛素mRNA表达量降低更明显,与各组比较有显著或极显著性差异(P<0.05或P<0.01)。中药组胰岛素mRNA表达量增加。 四、结论 1.情志刺激作为一个重要致病因素可诱发2型糖尿病的发生。该模型具有中戈满岁咨血糖升高,血清胰高血糖素升高,cAMP升高,胰岛素mRNA表达量下降的特征。其发病与情志应激引起的神经内分泌改变有关。 2.情志刺激可引起模型大鼠的一般状态、行为活动异常,反映了气机失调证的特征。 3.具有理气降逆散结作用的心身5号方可以明显改善神经内分泌系统的分泌功能,改善大鼠宏观行为活动异常。
【Abstract】 ObjectiveThe research is on the basis of the DM caused by many factors to discover the mechanisms of emotion causes on DM onset and the regulatory action to rats of composite formula Xinshen NO.5 which has the efficiency of regulating Qi lowering rebellious qi and dispersing accumulation.Methodon the basis of the theories of TCM about emotion caused diseases and the modern physiological-psychological medicine, the rat models of DM disease pattern were made by abdominal injecting little dosage of STZ and imposing psychophysiological experiment stimulation (crotation Restraint and crowding) simultaneously. The experiment animals were divided into five groups: normal group stress group STZ group stress+STZ groop and treatment group by Chinese herbs. Each group was checked the level of blood glucose serum insulin serum glucago cAMP and the transcriptional level of mRNA of insulin.And at the same time we observed the change of pancreas histomorphology and macro-performance.Result1 .Apparent and persistent blood glucose level increasing and serum insulin level decreasing were observed in the rats injected with STZ under emotional stimulation, the macro-performance the center check time crossing-check number . erecting number change were caused by the emotional stress. Compared with the other groups, the difference of these index were obvious(p<0.01or p<0.05). All those indicate that the rat model was accorded with the Qi-qi disorder syndrome type of DM.The level of blood glucose of treatment group by Chinese herb decreased and the level of serum insulin increased. And the change of the macro-performance of treatment group by Chinese herb is considerably light(p<0.01or p<0.05).2.Emotional stimulation can make the STZ rats’ serum glucagon level and the cAMP level of liver increased. The STZ plus stimulation group have the highest level (p<0.01orp<0.05).3.The transcriptional level of mRNA of insulin is decreased by the emotionalstimulation especially the STZ+stress group rats have the lowest level (p<0.01 or p<0.05). The treatment group level is added.Conclusion1.Emotional stimulation as an important pathogenic factor can induce DM. The model has the features of high level of blood glucose serum glucagon cAMP and low transcriptional level of mRNA of insulin .The onset is related with the change of neurosecretion system induced by emotional stimulation.2.Emotional stimulation can induce behavior disorder which reflects the features of Qi-ji disorder syndrome.3.Composite formula Xinshen No.5 can improve the action of neurosecretion system and the behavior reflection of rats obviously.
