清解攻下法治疗急性化脓性腹膜炎的临床与实验研究

【摘要】 急性化脓性腹膜炎(APP)危害性大。目前主要是以手术为主,配合抗生素治疗。随着分子生物学的发展,认识到APP产生的细菌移位和内毒素血症可导致全身炎症反应综合症,诱发多器官功能障碍。因此,本病除应消除腹腔炎症外,还应减轻全身炎症反应和器官功能损害。研究证实某些中药能降低外周血内毒素含量,调控某些炎症介子的过度表达。本临床研究以APP患者为对象,用清解攻下法,通过胃管内注入或灌肠给药,证明该法具有满意的效果,总有效率达89.13%,优于常规治疗对照组,无明显毒副作用。实验研究通过仿裴氏法大鼠腹腔内注入大肠杆菌,制造动物模型,证明该法具有抗炎、防止细菌移位、抑制炎症介子的过度表达、促进胃肠功能恢复等作用,并从中、西医理论的角度对有关问题进行了讨论。

【Abstract】 Acute pyoperitonitis (APP) is a kind of dangerously common encountered acute abdomen. At the present time, according to APP, the main therapy is operation, which is combined with antibiotics. Recently, following the development of molecular-biology, Bacterial translocation and endotoxemia, which resulting from APP, can lead to systemly inflammatory reaction syndrome (SIRS), and multi-organ dysfunctions (MODS). Some experimental researches have showed that some Chinese herbal drugs have notable effects in the treatment of acute abdomen, which can reduce endotoxin content in peripheral blood and regulate some inflammatory mediator’s overexpress. In the clinical part of this research, The principle of treatment is clearing away pathogenic heat and toxic waterial and purgation. Though nasal feeding or enema, the result shows exact and satisfactory curative effect. The total effective rate reach 89.13%, which is much better than the contrasting group by traditional routine treatment. And this therapeutic plan has no obvious toxic side effects. Through injecting bacillus coli into abdominal cavity to make animal model, the experimental part of research shows that this way has many functions, such as anti-inflammation, prohibiting bacterial translocation, inhibiting inflammatory mediators’ overexpress, and improving the resumption of gastrointestinal functions. This thesis discuss the relative problems of research in base of Chinese and modern medical theories.
