倾斜种植对种植体骨界面生物力学影响的三维有限元分析
Influence of Tilting Implantation on Biomechanical Distribution on Implant-bone Interface: A Three-Dimensional Finite Element Analysis

【摘要】 目的:建立包括颞下颌关节的下颌骨三维有限元模型,分析不同倾斜角度种植对种植体骨界面应力、应变及位移分布状况的影响,为临床设计应用提供理论参考依据。材料和方法:采用CT薄层扫描,计算机图像处理,建立无牙(牙合)下颌骨三维有限元模型。分别在第一磨牙区垂直及向舌侧倾斜10°、20°、30°植入一4.0mm×10mm圆柱形种植体,建立4个含种植体的下颌骨三维有限元模型。模拟咀嚼肌力加载,分析在正中咬合情况下种植体骨界面应力、应变及位移分布情况,并进行统计分析。结果:垂直种植时,应力集中于种植体颈部骨皮质,而种植体中份骨松质应力较小。随着倾斜角度的增大,种植体骨界面应力、应变及位移值均逐渐增大,应力值由集中于种植体颈部转变为底部。倾斜30°种植时,种植体骨界面应力值显著性增大(P<0.05),应变值及位移值也有明显的增大。结论:本实验结果表明种植体倾斜角度应小于30°,否则易引起骨组织微骨折及吸收破坏,影响长期成功率。
【Abstract】 Objective: Three-dimensional mathematical models of the mandible with temporomandibular joint was developed that was used to analyze the influence of different tilted angle on the distribution of the stress, strain and displacement on the implant-bone interface. Materials and Methods: CT scan and image processing were applied to construct a three-dimensional mathematical models of edentulous mandible . A 4. OmmXlOmm cylindrical implant was embedded in the first molar position, which was vertical and 10?^ 20?> 30?tilted lingually to occlusal plane respectively, to perform four three-dimensional finite element models with dental implants. A simulated force of masticatory muscles was loaded in the mathematical models, analysis of the stress, strain and displacement distribution on the implant-bone interface at central occlusion. Results: The stress concentration appeared around the neck of implants intergrated with the cortical bone, on the contrary , a little stress was found around the middle portion of implants intergrated with the cancellous bone. There was an increase in the magntitude of stress, strain and displacement as the angle increased, the stress concentration area transferred from the neck to the bottom around implants. When the embeded implant was angled 30? the stress ofimplant-bone interface increased significently(P<0.05), the magntitude of strain and displacement also increased obviously. Conclusions: The study revealed that the tilted angle of implants should be less than 30? or it would cause bone microfracture and absorption easily and reduce the rate of long-term success of dental implantation.
