基于定量CT技术分析不同体质量指数水平老年人体脂含量和心功能的相关性

【摘要】 目的 分析不同体质量指数(BMI)老年人群中基于定量CT(QCT)技术测量的脂肪含量与心脏功能的相关性。方法 选取2023年9月至2024年10月于河南省人民医院健康管理科行体检的老年人群为研究对象,按BMI分为肥胖组(n=61)、超重组(n=267)、正常体重组(n=247)、低体重组(n=12)。QCT测量内脏脂肪含量。通过彩色多普勒超声诊断仪检测心脏功能。收集血脂、血糖等代谢指标。分析内脏脂肪含量与心脏功能的相关性。结果 不同BMI老年人群心搏量(SV)、E峰、E/A值有差异(P<0.05)。随着BMI升高丙氨酸氨基转移酶(ALT)、血清尿酸(UA)、甘油三酯(TG)和空腹血糖逐渐升高,而高密度脂蛋白胆固醇(HDL-C)逐渐降低(P<0.05)。四组人群肝脏脂肪、总脂肪质量、总脂肪面积、总脂肪体积、内脏脂肪质量、内脏脂肪面积以及内脏脂肪体积均存在显著差异(P<0.05)。左心室射血分数(LVEF)、E峰、E/A、HDL-C、收缩压和舒张压与BMI、腰围、内脏脂肪面积等呈显著负相关(P<0.05),A峰则呈正相关(P<0.05)。结论 不同BMI水平老年人体脂含量和部分心功能指标存在差异,体脂含量均随着BMI升高逐渐增加,心功能随着BMI升高呈下降趋势;老年人群脂肪含量与心脏功能指标之间存在显著的相关性。

【Abstract】 Objective To analyze the correlation between fat content measured by quantitative CT(QCT)technology and cardiac function in elderly populations with different Body Mass Index(BMI).Methods The subjects were individuals who underwent health examinations at the Henan Provincial People s Hospital from September 2023 to October 2024,and were divided into obese group(n=61),overweight group(n=267),normal weight group(n=247),and underweight group(n=12) according to body mass index(BMI).QCT was used to measure visceral fat content.Cardiac function was assessed using a color Doppler ultrasound diagnostic device.Metabolic indicators such as blood lipids and blood sugar were collected.The relationship between visceral fat content and cardiac function was analyzed using Pea rson correlation analysis.Results There were differences in SV,E pea k,and E/A ratio among individuals with different BMI(P<0.05).As BMI increased,ALT,UA,TG,and fasting blood suga r gradually increased,while HDL-C gradually decreased(P<0.05).There were significant differences in liver fat,total fat mass,total fat a rea,total fat volume,visceral fat mass,visceral fat a rea,and visceral fat volume among the four groups(P<0.05),with fat content increasing as BMI increased.LVEF,E peak,E/A,HDL-C,systolic blood pressure,and diastolic blood pressure were significantly negatively correlated with BMI,waist circumference,and visceral fat area(P<0.05),while A peak was positively correlated(P<0.05).Conclusion There is a significant correlation between fat content and cardiac fu nction indicators in the elderly population.
