基于Apriori算法的甘肃省老年糖尿病住院患者共病情况分析
Analysis of Co-morbidities in Elderly Diabetic Hospitalized Patients in Gansu Province Based on Apriori Algorithm
【摘要】 目的 分析老年糖尿病患者的共病模式,为提高老年糖尿病的共病防治水平提供参考依据。方法收集甘肃省2022年公立医院就诊的59 592例老年糖尿病患者的住院病案首页,使用SPSS 26.0对不同性别患者的共病患病率作χ~2检验,使用SPSS Modeler 18.0软件对患者的共病情况作Apriori算法关联规则的挖掘。结果 59 592例老年糖尿病患者中,91.41%的患者存在共病,平均每人患有3.04种慢性病,女性患者的共病患病率高于男性(P<0.01)。女性比男性更易患高血压、肝脏疾病、血脂异常、胃肠道疾病、关节炎和情感及精神疾病(P<0.01),男性比女性更易患心脏病、脑卒中、肾脏疾病、慢性肺部疾病和肿瘤(P<0.01)。Apriori算法共挖掘出142条强关联规则,二元、三元和四元模式的关联规则分别有39条、78条和25条,主要指向高血压、心脏病、脑卒中、肾脏疾病、肝脏疾病、慢性肺部疾病、胃肠道疾病和眼部疾病。共病网络图提示高血压、心脏病和脑卒中的关联最紧密。结论 甘肃省老年糖尿病患者的共病患病率较高,形成了高血压、脑卒中和心脏病居于核心位置的共病模式,提示医疗机构和患者都应加强对糖尿病共病的预防及管理。
【Abstract】 Objectives To analyze the pattern of co-morbidity in elderly diabetic patients and provide a reference basis for improving the management of co-morbidity in elderly diabetes. Methods The first page of the inpatient medical records of 59,592 elderly diabetic patients attending public hospitals in Gansu Province in 2022 was collected, the prevalence of co-morbidities of patients of different genders was tested by chi-square test using SPSS 26.0, and the co-morbidities of patients were mined using the Apriori algorithm association rules using the SPSS Modeler 18.0 software. Results Of the 59,592 elderly diabetic patients, 91.41 % had co-morbidities, with an average of 3.04 chronic diseases per patient, and the prevalence of co-morbidities was higher in female patients than in males(P < 0.01). Women were more likely to suffer from hypertension, liver disease, dyslipidemia, gastrointestinal disease, arthritis, and emotional and mental disorders than men(P<0.01), and men were more likely to suffer from heart disease, stroke, kidney disease, chronic lung disease, and tumors than women(P<0.01).The Apriori algorithm mined a total of 142 strong association rules, and the binary, ternary, and quaternary patterns had 39, 78, and 25 association rules, respectively. rules, pointing to hypertension, heart disease, stroke, kidney disease, liver disease, chronic lung disease, gastrointestinal disease and eye disease. The co-morbidity network diagram suggests that hypertension, heart disease and stroke are most strongly associated. Conclusions The high prevalence of co-morbidities among elderly diabetic patients in Gansu Province had resulted in a pattern of co-morbidities in which hypertension, stroke and heart disease were at the center of the disease, suggesting that both healthcare providers and patients should strengthen the prevention and management of diabetes co-morbidities.
