脑灌注导向的重症患者血压管理的研究进展

【摘要】 脑灌注异常不仅影响患者短时间的脑功能,而且会导致不可逆的脑损伤,严重影响患者的预后。在重症医学科(ICU),患者可能常常伴有血压的异常、颅脑的疾病以及脑血管的调节功能异常,从而出现脑灌注的障碍,尤其是脑灌注不足。血压、颅内压和脑血管的调节功能,这三者决定了脑灌注。ICU患者存在不同类型的原发病,对于所有患者和每一类的患者,应该维持怎么样的血压较为合适,需要一个统一的标准;此外,对于每一例患者,根据脑灌注为导向的血压调控也均不相同,需要个体化的处理,所以在ICU的诊疗过程中对于上述参数的监测和整合尤为重要。

【Abstract】 Abnormal cerebral perfusion impacts not only short-term brain function but may also cause irreversible neurological deficits, significantly affecting patient prognosis. In intensive care units(ICU), patients often exhibit hemodynamic instability, intracranial pathologies, and impaired cerebrovascular autoregulation, which may result in cerebral perfusion disorders, particularly cerebral hypoperfusion. Cerebral perfusion is determined by three key factors: blood pressure, intracranial pressure, and cerebrovascular regulatory function. Given that ICU patients have various types of underlying primary diseases, there is a need for unified standards regarding optimal blood pressure maintenance for all patients and specific patient categories. Furthermore, cerebral perfusion-oriented blood pressure management requires individualized approaches for each patient. Therefore, the monitoring and integration of these parameters during ICU diagnostic and therapeutic processes are particularly crucial.
