【Abstract】 With socioeconomic development and improved living standards, the prevalence of diabetes and hypertension has risen annually. These conditions, characterized by high disability and mortality rates, coupled with low public awareness, treatment adherence, and control rates, have emerged as the most challenging global public health issues. In recent years, chronic disease management has gained global attention.
