【摘要】 目的 探讨祛腐生肌膏联合乌司他丁治疗肛周术后创面愈合效果及对炎症因子水平的影响。方法 选取2020年7月至2022年1月在桂林市中西医结合医院就诊并行肛周疾病手术后的患者为研究对象，随机分为对照组和试验组。两组患者均给予常规清创治疗及乌司他丁治疗，试验组加祛腐生肌膏治疗。比较两组患者的创面愈合疗效、中医证候积分、炎症因子水平、生长因子水平和治疗安全性。结果 研究共纳入患者116例，试验组和对照组各58例。试验组总有效率（91.38%）高于对照组（75.86%），差异有统计学意义（P <0.05）。治疗后，试验组改为中医证候积分及白细胞介素-17A(IL-17A)、C反应蛋白（CRP）、血清淀粉样蛋白A(SAA)水平均低于对照组（P <0.05）；血管内皮生长因子受体1(VEGFR1)、成纤维细胞生长因子受体（FGFR）、转化生长因子-β1(TGF-β1)及肛门功能指标均高于对照组（P <0.05）。治疗期间两组患者不良反应发生率分别为13.79%和8.62%，差异无统计学意义（P> 0.05）。结论 祛腐生肌膏联合乌司他丁治疗肛周术后创面愈合的效果显著，可改善患者中医证候积分，降低炎症因子，上调生长因子，且安全性良好。更多还原

【Abstract】 Objective To investigate the effect of Qufu Shengji ointment combined with ulinastatin in the treatment of wound healing a?ter perianal surgery and its effect on the level of inflammatory factors. Methods Patients who underwent perianal surgery in Guilin Hospital of Integrated Traditional Chinese and Western Medicine from July 2020 to January 2022 were randomly divided into control group and test group. The patients in both groups were treated with conventional debridement therapy and ulinastatin, and the test group was treated with Qufu Shengji ointment. The wound healing efficacy, TCM symptom score,inflammatory factor level, growth factor level and treatment safety of the two groups were compared. Results A total of 116 patients were included in the study, including 58 patients in the test group and 58 in the control group. The total effective rate of the test group(91.38%)was higher than that of the control group(75.86%), and the difference was statistically significant(P<0.05). A?ter treatment, the TCM syndrome score levels of interleukin-17A(IL-17A), C-reactive protein(CRP) and serum amyloid A(SAA) in the test group were lower than those in the control group(P<0.05). The levels of vascular endothelial growth factor receptor 1(VEGFR1), fibroblast growth factor receptor(FGFR) and transforming growth factor-β1(TGF-β1) were higher than those in the control group(P<0.05). The anal function index was higher than that of the control group(P<0.05). The incidence of adverse reactions between the two groups was 13.79% and 8.62%, respectively, and the difference was not statistically significant(P>0.05). Conclusion The effect of Qufu Shengji ointment combined with ulinastatin in the treatment of wound healing a?ter perianal surgery is significant, which can improve the TCM syndrome, reduce inflammatory factors, and upregulate growth factors, and has good safety.更多还原
