脑梗死患者血清Hcy联合MPV检测与颈动脉血管斑块的相关性研究

【摘要】 目的 探讨脑梗死患者血清同型半胱氨酸(Hcy)和外周血平均血小板体积(MPV)与颈动脉血管斑块的相关性,并分析其临床应用价值。方法 2020年10月~2021年10月齐齐哈尔医学院第三附属医院神经内科住院的103例脑梗死患者为试验组(根据超声颈动脉血管斑块结果确定66例颈动脉斑块亚组和37例颈动脉正常亚组),同期选取109例健康体检者为对照组。各组均检测血清同型半胱氨酸(Hcy)和平均血小板体积(MPV)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等代谢指标,并分析其统计学差异。结果 试验组患者MPV、TC、TG、LDL-C、Hcy水平显著高于对照组(P <0.05),HDL-C水平显著低于对照组(P <0.05)。颈动脉斑块亚组MPV、TC、TG、LDL-C、Hcy水平显著高于颈动脉正常亚组(P <0.05)。Spearman相关性分析显示,脑梗死与MPV、TC、TG、LDL-C、Hcy表达正相关(P <0.05),与HDL-C负相关(P <0.05);脑梗死患者MPV、TC、TG、LDL-C、Hcy表达与颈动脉斑块程度正相关(P <0.05)。结论 血清Hcy、MPV、TC、TG、LDL-C和HDL-C水平与颈动脉斑块密切相关,可作为临床筛查脑梗死的检测指标。

【Abstract】 Objective To investigate the correlation between serum homocysteine( Hcy) combined with mean platelet volume(MPV) in peripheral blood and carotid artery plaque in patients with cerebral infarction, and to analyze its clinical application value. Methods From October 2020 to October 2021, 103 patients with cerebral infarction who were hospitalized in the Department of Neurology, The Third Affiliated Hospital of Qiqihar Medical College were selected as the test group(66 carotid artery plaque subgroups and 37 carotid artery normal subgroups were determined according to the results of ultrasound carotid plaques), and 109 healthy subjects were selected as the control group during the same period. Serum homocysteine(Hcy), mean platelet volume(MPV), total cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C), and low-density lipoprotein cholesterol(LDL-C) were detected in each group and analyze their statistical differences.Results The levels of MPV, TC, TG, LDL-C and Hcy in the test group were significantly higher than those in the control group(P < 0.05), and the levels of HDL-C were significantly lower than those in the control group(P < 0.05). The levels of MPV, TC, TG, LDL-C and Hcy in the carotid artery plaque subgroup were significantly higher than those in the carotid artery normal subgroup(P < 0.05). Spearman correlation analysis showed that cerebral infarction was positively correlated with the expression of MPV, TC, TG, LDL-C and Hcy(P < 0.05), and negatively correlated with HDL-C(P < 0.05). The expression of MPV、TC、TG、LDL-C、Hcy was positively correlated with the degree of carotid artery plaque(P < 0.05). Conclusion Serum levels of Hcy, MPV, TC, TG, LDL-C and HDL-C are closely related to carotid artery plaques and can be used as detection indicators for clinical screening of cerebral infarction.
