子宫内膜癌分子分型与临床病理特征分析
Molecular classification and clinicopathological features of endometrial carcinoma
【摘要】 目的 探讨子宫内膜癌分子分型与临床病例特征。方法 收集河北医科大学附属沧州市中心医院行子宫内膜癌手术治疗的患者46例,利用免疫组化和Sanger测序,按WHO推荐方法进行分子分型,分析各亚组临床病理特征。结果 纳入46例子宫内膜癌,其中宫内膜样腺癌41例、浆液性癌4例、去分化子宫内膜癌1例。2例POLEmut均为exon9突变:c.857C>G(P.P286R),其中1例伴p53异常。20例MMRd亚型中,15例MLH1/PMS2同时缺失,均为MLH1启动子甲基化导致。4例P53abn亚型中,以错义突变表达模式为主。NSMP亚型均为宫内膜样腺癌。结论 通过分析各分子亚组组织学特征,肿瘤形态可以提供子宫内膜癌分子分型的线索,只有将临床病理因素和分子分型结合起来,才能为临床预后评估提供重要依据。
【Abstract】 Objective To investigate the molecular classification and clinical case characteristics of endometrial cancer.Methods Forty-six cases of endometrial cancer underwent surgical treatment in Cangzhou Central Hospital were collected using immunohistochemistry and Sanger sequencing. According to the method recommended by WHO, the molecular typing was carried out, and the clinicopathological characteristics of each subgroup were analyzed. Results A total of 46 cases of endometrial cancer were included, Among them, there were 41 cases of endometrioid adenocarcinoma, 4 cases of serous carcinoma, and 1 case of dedifferentiated endometrial carcinoma. The 2 cases of POLEmut both had exon9 mutation: c.857C>G(P.P286R), and 1 case had abnormal p53. Among the 20 cases of MMRd subtype, 15 cases of simultaneous deletion of MLH1/PMS2 were all caused by methylation of MLH1 promoter. Among the 4 cases of p53abn subtype, the expression pattern of missense mutation was dominant. The NSMP subtypes were all endometrioid adenocarcinomas. Conclusion By analyzing the histological characteristics of each molecular subgroup, tumor morphology can provide clues to the molecular classification of endometrial cancer. In the future, only by combining clinicopathological factors with molecular classification can we provide an important basis for clinical prognosis assessment.
