精神分裂症代谢综合征和神经认知的关系

【摘要】 目的 横断面研究精神分裂症患者代谢综合征(MS)及其各组分和神经认知的关系。方法 根据DSM-IV-TR标准纳入精神分裂症患者117名,收集社会人口信息、临床特征、人体测量、血清测试、C反应蛋白(CRP)和神经认知测量数据。分别以MS作为二分法变量(是/否)、连续变量(MS每个组成)进行回归分析,以探索可能与神经认知相关的代谢参数。结果 Logistic回归分析显示MS和炎症CRP(β=0.447,P=0.001)、氯丙嗪当量(β=0.001,P=0.040)及延时记忆(β=-0.034,P=0.031)存在显著相关。多元线性回归分析显示除视觉广度外,其余认知功能方面均与MS的不同组分存在相关性(P<0.05)。结论 MS与认知功能有关,这些发现表明MS的改变可能参与精神分裂症患者认知损害的病理机制。

【Abstract】 Objective To study the relationship between metabolic syndrome(MS) and its components and neurocognition in patients with schizophrenia. Methods A total of 117 patients with schizophrenia were enrolled according to DSM-IV-TR criteria. Data of socio-demographic information, clinical characteristics, anthropometry, serum test, C-reactive protein(CRP) and neurocognitive measurements were collected. Regression analyses were performed using MS as dichotomous variables(yes/no) and continuous variables(MS per component) to explore metabolic parameters that might be associated with neurocognition. Results Logistic regression analysis showed that MS were significantly correlated with inflammatory CRP(β=0.447, P=0.001), chlorpromazine equivalent(β=0.001, P=0.040) and delayed memory(β=-0.034, P=0.031). Multiple linear regression analysis showed that all aspects of cognitive function except visual span were correlated with different components of MS(P<0.05). Conclusion MS are associated with cognitive impairment, and these findings suggest that changes in MS may be involved in the pathological mechanism of cognitive impairment in patients with schizophrenia.
