慢性病患者医患共享决策参与意愿及影响因素研究——以哈尔滨市某三甲医院为例
Study on the willingness of patients with chronic diseases to participate in shared decision making willingness and its influencing factors: based on data from a tertiary hospital in Harbin
【摘要】 目的 探索慢性病患者共享决策参与意愿的影响因素,为进一步提升患者的共享决策参与意识提供建议。方法 采用自主设计慢性病患者共享决策与调查问卷,用随机抽样方法,在慢性病发病率较高的哈尔滨市开展问卷调查,共回收有效问卷231份。通过卡方检验和二元逻辑Logistic回归方程对患者参与共享决策的意愿及影响因素进行分析。结果 231名调查对象中,75.8%的患者有意愿进行医患共享决策;Logistic回归分析结果显示,慢性病患者对不良情绪的调控能力(OR=0.171,95%CI:0.052,0.569)、对医患共享决策了解度(OR=22.260,95%CI:7.048,70.307)、有无慢性病家族史(OR=0.377,95%CI:0.145,0.979)及医务人员共情能力(OR=37.132,95%CI:5.658,243.691)均会对共享决策意愿产生影响。结论 慢性病患者共享决策意愿整体上较高;加强医患共享决策的宣传力度、关注患者的患病家族史情况和性格特点、提升医务人员的共情能力和人文素养,对于医患共享模式的实施至关重要。
【Abstract】 Objective To explore the willingness of chronic disease patients to participate in shared decision making and providing suggestions and its influencing factors to further enhance patients’ awareness of shared decision making participation. Methods A self-designed questionnaire to explore shared decision making among patients with chronic diseases was used. The survey was conducced in Harbin, a city with a high prevalence of chronic diseases by employ random sampling, and 231 valid questionnaires were collected. The patients’ willingness to participate in shared decision-making and the influencing factors was analyzed by Chi-square test and binary Logistic regression equation. Results Among the 231 respondents, 75.8%of patients were willing to engage in shared decision making; Logistic regression analysis results showed that chronic disease patients’ ability to regulate adverse emotions(OR=0.171,95%CI: 0.052,0.569), knowledge of shared decision making between doctors and patients(OR=22.260, 95%CI: 7.048,70.307), presence of family history of chronic disease(OR=0.377, 95%CI: 0.145,0.979) and medical staff empathy(OR=37.132, 95%CI: 5.658,243.691) all had an impact on willingness to share decision making. Conclusion Patients with chronic diseases have an overall high willingness to share decision making; Strengthening the publicity of shared decision making between doctors and patients, paying attention to patients’ family history of illness and personality traits, and improving the empathy and humanistic skills of medical staff are crucial to the implementation of the shared decision making.
