【摘要】 目的 观察腕踝针联合穴位注射对急性腰扭伤患者的临床疗效，并探讨其对炎症因子、α-颗粒膜糖蛋白及MMP-3表达的影响。方法 将90例急性腰扭伤患者按随机数字表法分为对照组与观察组各45例，对照组给予西医常规治疗，观察组在常规治疗基础上给予腕踝针联合穴位注射，比较两组的临床效果及对炎症因子、α-颗粒膜糖蛋白及MMP-3表达的影响。结果 观察组总有效率为91.11%，高于对照组的71.11%(P <0.05)。与治疗前比较，两组治疗后腰痛、活动、行走、肿胀及瘀斑等积分明显降低，且观察组积分明显低于对照组（P <0.05）。两组治疗后RMDQ和ROM评分明显降低；与对照组治疗后比较，观察组RMDQ和ROM评分明显低于对照组（P <0.05）。两组治疗后血清TNF-α、IL-10和CRP水平明显降低，与对照组比较，观察组上述指标明显低于对照组（P <0.05）。两组治疗后α-颗粒膜糖蛋白及MMP-3水平明显降低，与对照组比较，观察组降低更显著（P <0.05）。结论 腕踝针联合穴位注射对急性腰扭伤患者疗效确切，可显著改善中医症状，提高腰椎功能和腰椎活动度，可能与调节炎症因子、α-颗粒膜糖蛋白及MMP-3水平有关。更多还原

【Abstract】 Objective:To observe the clinical efficacy of wrist-ankle acupuncture combined with acupoint injection on patients with acute lumbar sprain,and to explore its effect on the expression of inflammatory factors,α-granular membrane glycoprotein and MMP-3. Methods:A total of 90 patients with acute lumbar sprain were randomly divided into the control group(45 cases)and observation group(45 cases). The control group was given conventional western medicine treatment while the observation group was given wrist-ankle acupuncture and acupoint injection on the basis of conventional treatment. The clinical effects,their influence on the expression of inflammatory factors,α-granular membrane glycoprotein and MMP-3 of the two groups were compared. Results:The total effective rate of the observation group was 91.11%,which was higher than 71.11% of the control group(P < 0.05).Compared with before treatment,the scores of low back pain,activity,walking,swelling and ecchymosis were significantly reduced in the two groups after treatment;compared with the control group after treatment,the scores of the observation group were significantly lower than those of the control group(P < 0.05). The RMDQ and ROM scores of the two groups were significantly reduced after treatment;compared with the control group after treatment,the RMDQ and ROM scores of the observation group were significantly lower than those of the control group(P <0.05). Serum levels of TNF-α,IL-10 and CRP were significantly reduced in the two groups after treatment;compared with the control group after treatment,the above indicators in the observation group were significantly lower than those in the control group(P < 0.05). The levels of α-granular membrane glycoprotein and MMP-3 were significantly reduced after treatment in the two groups;compared with the control group after treatment,the above indicators in the observation group were significantly lower than those in the control group(P < 0.05). Conclusion:Wrist-ankle acupuncture combined with acupoint injection has definite effect on patients with acute lumbar sprain. It can significantly improve the symptoms of TCM,improve lumbar function and lumbar spine mobility,which may be related to the levels of regulations with inflammatory factors,α-granular membrane glycoprotein and MMP-3.更多还原
