天津某医院急性心肌梗死患者发病情况及其相关危险因素分析

【摘要】 目的 对急性心肌梗死患者发病时基本情况、血糖、血脂和尿酸代谢进行回顾性研究,分析其相关性。方法收集2017-2019年南开大学附属第四中心医院胸痛中心收治的急性心肌梗死住院患者相关数据,包括性别、患病年龄、月份、昼夜等一般情况,血清总胆固醇、甘油三脂、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、空腹血糖和血尿酸等实验室指标。将患病年龄范围分13个年龄段,根据代谢异常组合分组,分析代谢异常与上述一般情况分布差异的关系。结果 本研究共纳入736例患者,男性554例(75.27%),女性182例(24.73%),患者年龄范围29-99岁,平均发病年龄(64.39±11.81)岁,男性(62.15±11.15)和女性(71.42±11.04)患者平均发病年龄间差异具有显著性(t=-10.758,P=0.000)。各年龄段分布中男性和女性患者间(χ~2=78.798,P=0.000),男性患者间(χ~2=423.869,P=0.000)及女性患者间(χ~2=99.895,P=0.000)分布差异均有显著性;在12个月间男性患者和女性患者之间分布差异无显著性(χ~2=4.54,P=0.951)、男性患者间(χ~2=30.878,P=0.001)分布差异有显著性,女性患者间(χ~2=15.538,P=0.159)分布差异无显著性;24小时中男性和女性患者间分布差异无显著性(χ~2=13.751,P=0.934),男性患者间(χ~2=77.497,P=0.000)分布差异有显著性,女性患者间(χ~2=26.977,P=0.257)分布差异无显著性,男性患者在8:00-11:00、15:00-16:00、21:00左右是发病高峰时段。男性患者间(χ~2=125.903,P=0.002)代谢指标完全正常者与代谢指标异常者分布差异有显著性,女性患者间(χ~2=72.772,P=0.387)代谢指标完全正常者与代谢指标异常者分布差异无显著性。结论 急性心肌梗死发病存在年龄和性别差异,老年多见,男性多于女性,在男性存在昼夜和季节节律,且男性AMI与血糖血脂代谢异常有关,与尿酸未发现相关,血脂异常以HDL-C异常多见。女性AMI发生未发现与代谢异常相关。

【Abstract】 Objective A retrospective study on the general condition,metabolism of blood glucose,blood lipid and uric acid of patients with acute myocardial infarction was conducted to analyze their correlation.Methods The research requires data from acute myocardial infarction inpatients in the fourth Central Hospital Affiliated to Nankai University from 2017to 2019which includes general conditions such as gender,age of illness,month、time of onset and laboratory indicators such as TC,TG,LDH-C,HDL-C,blood glucose and blood uric acid.The samples were divided into 13aage brackets and then grouped according to the abnormal combination of metabolism.Finally,the research analyzes the relationship between abnormal metabolism and distributions of general conditions above mentioned.Results A total of 736patients were enrolled in the study,including 554males(75.27%)and 182females(24.73%).The age of patients ranges from 29to 99and the average age of onset was(64.39±11.81)years old.There exists significant difference between male(62.15±11.15)and female(71.42±11.04)patients in average age of onset(t=-10.758,P=0.000);There was significant difference between each age group in the gender distribution(χ~2=78.798,P=0.000),male patients(χ~2=429.869,P=0.000),female patients(χ~2=99.895,P=0.000);There was no significant difference in the distribution of the month of onset in all genders(χ~2=4.54,P=0.951)and in female patients(χ~2=15.538,P=0.159)but there exists significant difference in male patients(χ~2=30.878,P=0.001);There was no significant difference in the distribution of the time of onset in all genders(χ~2=13.751,P=0.934)and in female patients(χ~2=26.977,P=0.257),but there exists significant difference in male patients(χ~2=77.497,P=0.000)and 8:00-11:00,15:00-16:00,21:00were peak time of onset;There was significant difference on male patients of normal or abnormal metabolic index(χ~2=125.903,P=0.002)and there was no significant difference on female patients of normal or abnormal metabolic index(χ~2=72.772,P=0.387).Conclusion There exist differences on onset of acute myocardial infarction in age and sex.The elderly are more likely to suffer an onset.There are circadian and seasonal rhythms in male patients,and male patients with AMI are related to abnormal blood glucose and lipid metabolism.Abnormal HDL-C is more common in abnormal metabolism.The occurrence of female patients with AMI was not found to be related to metabolic abnormalities.
