HIV感染伴尖锐湿疣的HPV分型特点探讨

【摘要】 目的 探讨人免疫缺陷病毒(HIV)感染伴尖锐湿疣(CA)的人乳头瘤病毒(HPV)分型特点。方法 544例尖锐湿疣(CA)患者,根据HIV检测是否阳性分为HIV阳性组(261例)和HIV阴性组(283例)。检测两组患者的HPV分型。比较两组患者HPV低危型、高危型检出情况, HPV各亚型分布情况, HPV单一感染和多重感染情况。结果 HIV阳性组患者HPV高危型检出率57.1%高于HIV阴性组的30.0%,低危型检出率24.1%低于HIV阴性组的51.2%,差异具有统计学意义(P<0.05);两组HPV高危型合并低危型检出率比较差异无统计学意义(P>0.05)。HPV低危型HPV11在HIV阳性组和HIV阴性组患者中的检出率均最高, HIV阳性组HPV11检出率56.7%明显高于HIV阴性组的37.8%,差异具有统计学意义(P<0.05);HPV高危型HPV16在HIV阳性组和HIV阴性组患者中的检出率均最高,其次为HPV58, HIV阳性组HPV16、HPV58检出率分别为28.0%、17.6%,明显高于HIV阴性组的18.0%、11.7%,差异具有统计学意义(P<0.05)。HIV阳性组患者多重感染率72.0%高于HIV阴性组的48.1%,单一感染率28.0%低于HIV阴性组的51.9%,差异具有统计学意义(P<0.05)。结论 HIV阳性伴CA患者HPV高危型、多重感染几率明显高于HIV阴性患者,从而加重HPV感染导致的病变,在临床上应引起重视。

【Abstract】 Objective To discuss the human papillomavirus(HPV) typing characteristics of human immmunodeficiency virus(HIV) infection with condyloma acuminatum(CA). Methods 544 patients with condyloma acuminatum(CA) were divided into HIV-positive group(261 cases) and HIV-negative group(283 cases) according to whether the HIV test was positive or not. HPV typing was detected in the two groups.The detection of HPV low-risk and high-risk types, the distribution of HPV subtypes, HPV single infection and multiple infections were compared between the two groups. Results The detection rate of HPV high-risk type 57.1% in HIV-positive group was higher than 30.0% in HIV-negative group; the detection rate of low-risk type 24.1% in HIV-positive group was lower than 51.2% in HIV-negative group; the differences were all statistically significant(P<0.05). There was no statistically significant difference in the detection rate of HPV high-risk type combined with low-risk type between the two groups(P>0.05). HPV low-risk HPV11 had the highest detection rate in both HIV-positive and HIV-negative patients. The detection rate of HPV11 in the HIV-positive group was 56.7%, which was significantly higher than 37.8% in the HIV-negative group, and the difference was statistically significant(P<0.05). HPV high-risk type HPV16 had the highest detection rate in both HIV-positive and HIV-negative patients, followed by HPV58. The detection rates of HPV16 and HPV58 in HIV-positive group were 28.0% and 17.6%, which were significantly higher than 18.0% and 11.7% in HIV-negative group, and the differences were all statistically significant(P<0.05). In the HIV-positive group, the multiple infection rate 72.0% was higher than 48.1% in the HIV-negative group, and the single infection rate 28.0% was lower than 51.9% in the HIV-negative group, and the differences were all statistically significant(P<0.05). Conclusion HIV-positive patients with CA are significantly more likely to have HPV high-risk types and multiple infections than HIV-negative patients, thus aggravating the lesions caused by HPV infection, which should be paid attention to in clinical practice.
