孕早期唐氏综合征血清学筛查指标与新生儿体质量的相关性分析
Correlation analysis between serological screening indexes of Down’s syndrome and neonatal body weight in early pregnancy
【摘要】 目的 分析孕早期唐氏综合征(DS)血清学筛查指标与新生儿结局的相关性。方法 回顾性分析2016年1—12月于福建医科大学附属福州市第一医院行孕早期血清学筛查并分娩的786名产妇作为临床资料,所有产妇于孕11~13+6周均接受DS血清学检测。根据产妇的血清学筛查风险值分为高风险组(n=260)和低风险组(n=526),比较两组产妇孕早期DS血清学筛查指标和新生儿结局情况,并分析DS血清学筛查指标与新生儿结局的相关性。结果 高风险组新生儿分娩孕龄及新生儿出生体质量均低于低风险组,差异有统计学意义(P<0.05)。高风险组产妇血清游离人绒毛膜促性腺激素(β-hCG)、β-hCG中位数的倍数(MOM)、游离雌三醇(uE3)和uE3-MOM 水平均高于低风险组,差异有统计学意义(P<0.05)。Spearman相关分析显示,孕早期血清uE3水平与新生儿出生体质量呈正相关(r=0.918,P<0.001)。多元Logistic回归分析显示,唐氏筛查高风险(OR=2.07,95%CI:1.89~2.11)和早期血清uE3水平(OR=1.23,95%CI:1.05~1.94)与新生儿体质量有关,可作为预测不良妊娠结局的辅助指标。结论 孕早期血清学uE3水平对预测新生儿出生体质量有参考价值,具有重要的临床意义。
【Abstract】 Objective To analyze the correlation between serological screening indexes of Down’ s syndrome (DS) and neonatal body weight in early pregnancy. Methods A retrospective analysis of 786 puerperae who underwent first-trimester serological screening and gave birth in Fuzhou First Hospital Affiliated to Fujian Medical University from January to December 2016 were used as clinical data.. All pregnant women received DS serological test at 11-13+6weeks of pregnancy. The patients were divided into high-risk group (n=260) and low-risk group (n=526) according to the risk value of maternal serological screening. The DS serological screening indexes and neonatal outcomes in the early pregnancy of the two groups were compared, and the correlation between DS serological screening indexes and neonatal outcomes were analyzed. Results The gestational age and birth weight of newborns in the high-risk group were lower than those in the low-risk group, the difference was statistically significant (P<0.05). The levels of serum β human chorionic gonadotropin (β-hCG) and β-hCG-multiple of the median (MOM), free estriol (uE3) and uE3-MOM in high-risk group were significantly higher than those in low-risk group, the difference was statistically significant (P<0.05). Spearman correlation analysis showed that the level of serum uE3in early pregnancy was positively correlated with the birth weight of newborns (r=0.918, P<0.001). When adjusted Logistic regression analyses revealed that low birth weight were significantly associated with DS high-risk (OR=2.07, 95%CI:1.89-2.11) and serum uE3level (OR=1.23, 95%CI:1.05-1.94). Conclusion The level of serum uE3in early pregnancy has reference value and important clinical significance in predicting the birth weight of newborns.
