BI-RADS分类联合各向异性因子对乳腺肿块的诊断价值
Diagnostic Value of BI-RADS Classification Combined with Anisotropy Factors for Breast Masses
【摘要】 目的利用剪切波弹性成像技术评估乳腺肿块的各向异性,探讨各向异性因子(AF)及其联合BI-RADS分类对乳腺肿块的诊断价值。方法收集102例患者共106个肿块在最大径线及垂直切面的弹性定量指标,分析AFs及其联合BI-RADS分类对乳腺良恶性肿块的诊断价值。结果恶性肿块的AFs明显高于良性肿块(P<0.05)。BI-RADS分类及AFratio诊断的灵敏度、特异度、准确度、Kappa值分别为85.4%、75.9%、80.2%、0.605及72.9%、86.2%、80.2%、0.597。二者联合诊断的灵敏度、特异度、准确度、Kappa值分别为83.3%、89.7%、86.8%、0.733。联合诊断的Kappa值最高,特异度较单一BI-RADS分类高(χ~2=3.876,P=0.049<0.05)。结论 AF可作为乳腺癌的诊断指标,AFratio联合BI-RADS分类可提高乳腺肿块诊断的特异度及可靠性,有一定的应用价值,值得进一步研究。
【Abstract】 Objective To evaluate the anisotropy of breast masses using shear wave elastography and to explore the diagnostic value of anisotropy factor and it combined with BI-RADS classification for breast masses. Methods The SWE quantitative indicators of 102 patients with 106 tumors were recorded in the largest radial section and its vertical section, then AFs were calculated and the diagnostic value of these masses were analyzed. Combining with the results of BI-RADS, the value of the two for breast cancer diagnosis were explored. Results The AFs of malignant masses were significantly higher than benign masses(P<0.05). The sensitivity, specificity, accuracy, and Kappa values of BI-RADS and AFratio were 85.4%, 75.9%, 80.2%, 0.605, and 72.9%, 86.2%, 80.2%, 0.597, respectively. These indexes of the two combined diagnosis methods were 83.3%, 89.7%, 86.8% and 0.733. The Kappa value of the combination is the highest, and the specificity is higher than that of the single BI-RADS grade(χ~2=3.876, P=0.049<0.05). Conclusions AF can be used as a diagnostic indicator for breast cancer. AFratio combined with BI-RADS classification can improve the specificity and reliability of breast mass diagnosis. It has certain application value and is worthy of further research.
