复合手术室中治疗脑动静脉畸形的效果分析与评价

【摘要】 目的探讨在复合手术室中对脑动静脉畸形患者进行治疗的效果。方法选取2016年1月—2018年5月来该院接受治疗的20例脑动静脉畸形患者进行研究,均在复合手术室中完成手术治疗,结合术前患者评估结果,分为有5例患者行开颅切除术与术中脑血管造影,另15例患者行血管内栓塞术与开颅切除术。分析患者手术情况、临床疗效、术后并发症发生情况与随访结果。结果 20例患者手术时间在4.0~8.3 h左右,平均手术时间为(5.9±1.1)h;15例行血管内栓塞术与开颅切除术患者在经过治疗后,畸形血管团体积缩小了46.7%~63.9%;术后无一例患者出现手术死亡,且未发生与造影有关的并发症;有1例患者(5%)发生肢体偏瘫,有1例患者(5%)术后出现脑梗死,直径未超过2 cm,无一例患者出现颅内感染、手术切口感染或颅内出血等并发症;术后随访结果显示,无一例患者出现迟发性癫痫症状;术后6个月对患者实施DSA检查或CTA检查,检查结果显示患者脑动脉畸形血管团均完整切除,无残留现象。结论在复合手术室中对脑动静脉畸形患者进行治疗具有显著疗效,不仅能够提高手术全切率,且术后并发症发生率较低,为脑动静脉畸形患者提供了有效且安全的一种新型治疗平台,值得推广应用。

【Abstract】 Objective To investigate the effect of treatment for patients with cerebral arteriovenous malformationin a compound operating room. Methods Selected 20 patients with cerebral arteriovenous malformations who came to the hospital for treatment from January 2016 to May 2018 for research. They all completed the surgical treatment in the compound operating room. Combined with the preoperative patient evaluation results, they were divided into 5 patients who underwent craniotomy with intraoperative cerebral angiography, the other 15 patients underwent endovascular embolization and craniotomy. Analyzed the patient’s surgical status, clinical efficacy, postoperative complications and follow-up results. Results The operation time of 20 patients was about 4.0-8.3 h, and the average operation time was(5.9±1.1)h. After treatment, the volume of the deformed blood vessel group was reduced in 15 patients who underwent endovascular embolization and craniotomy 46.7%-63.9%; no postoperative death occurred in 1 patient, and no angiography-related complications occurred; 1 patient(5%) had limb hemiplegia, and 1 patient(5%) had brain after operation. Infarction, diameter less than 2 cm, no patient had complications such as intracranial infection, surgical incision infection, or intracranial hemorrhage; postoperative follow-up results showed that no patient had delayed epilepsy symptoms; 6 months after surgery, the patient underwent DSA examination or CTA examination, and the examination results showed that the cerebral artery malformation vascular masses of the patient were completely removed, and there was no residual phenomenon. Conclusion The treatment of patients with cerebral arteriovenous malformations in the compound operating room has a significant effect. It can not only increase the total surgical resection rate, but also has a lower incidence of postoperative complications. It provides effective and effective treatment for patients with cerebral arteriovenous malformations. A safe new type of treatment platform, worthy of promotion and application.
