Braden评分预测重症缺血性脑卒中患者医院获得性压力性损伤的可行性
The Feasibility of Braden Score for Predicting Hospital-acquired Stress Injury in Patients with Severe Ischemic Stroke
【摘要】 目的 评估Braden评分预测重症缺血性脑卒中患者医院获得性压力性损伤(hospital-acquired pressure injuries,HAPrIs)发生率的可行性。方法 分析2019年1月—2021年1月入住该院的重症缺血性脑卒中患者210例,根据患者是否有HAPrIs分为观察组(28例)和对照组(182例)。收集比较两组患者的Braden评分、临床资料,用ROC曲线分析Braden评分对预测重症缺血性脑卒中患者医院获得性压力性损伤的可行性。结果 观察组NIHSS评分(19.6±5.1)分,明显高于对照组的(17.5±3.9)分,差异有统计学意义(t=-2.538, P=0.012);观察组的Braden评分(12.1±2.5)分,明显低于对照组的(13.6±2.3)分,差异有统计学意义(t=3.176, P=0.002);采用ROC计算曲线下面积为0.721 (95%CI=0.678~0.877),以13分作为节点,敏感性为73.2%,特异性为74.1%。结论 重症缺血性脑卒中患者的HAPrIs与Braden评分存在相关性,更低的Braden评分指向更高的HAPrIs风险。以Braden评分13分来预测重症缺血性脑卒中的HAPrIs发生率,敏感性为73.2%,特异性为74.1%。
【Abstract】 Objective To evaluate the feasibility of Braden score predicting the incidence of hospital-acquired pressure injuries(HAPrIs)in patients with severe ischemic stroke.Methods To analyze 210 patients with severe ischemic stroke admitted to the hospital from January 2019 to January 2021.According to whether the patients had HAPrls,they were divided into observation group(28 cases)and control group(182 cases).The Braden score and clinical data of the two groups of patients were collected and compared,and the ROC curve analysis of the Braden score was used to predict the feasibility of hospital-acquired pressure injury in patients with severe ischemic stroke.Results The NIHSS score of the observation group was(19.6±5.1)points,which was significantly higher than the control group’s(17.5±3.9) points,the difference was statistically significant(t=-2.538,P=0.012).The Braden score of the observation group was(12.1±2.5) points,which was significantly lower than the(13.6±2.3) points of the control group,the difference was statistically significant(t=3.176,P=0.002).The area under the curve calculated by ROC was 0.721(95% CI=0.678~0.877),with 13 points as the cut-off point,the sensitivity was 73.2%,and the specificity was 74.1%.Conclusion There is a correlation between HAPrIs and Braden score in patients with severe ischemic stroke.A lower Braden score points to a higher risk of HAPrls.Using the Braden score of 13 to predict the incidence of HAPrIs in severe ischemic stroke,the sensitivity was 73.2% and the specificity was 74.1%.
