痤疮丙酸杆菌生物膜形成与抗菌药物耐药的相关性分析

【摘要】 目的分析痤疮丙酸杆菌生物膜形成与抗菌药物耐药的相关性。方法从2019年1—6月复旦大学附属华山医院痤疮患者皮损处分离、鉴定获得44株痤疮丙酸杆菌。采用琼脂稀释法进行药敏试验,结果按美国临床和实验室标准化协会(CLSI)2018年标准判读,根据药敏试验结果将菌株分为红霉素耐药株、克林霉素耐药株、克林霉素-红霉素双重耐药株。体外培养至96 h及120 h,结晶紫染色检测生物膜生物量(D570 nm)、刃天青染色检测生物膜内活菌数量(560Ex/590Em)、激光共聚焦显微镜观察生物膜结构。结果培养至96 h及120 h时,耐药株的D570 nm、560Ex/590Em均明显高于敏感株(P<0.05)。当培养至120 h时,克林霉素-红霉素双重耐药株能牢固附着于玻璃底荧光皿底部,形成均匀的膜状结构,生物膜内部结构排列有序。而敏感株形成的生物膜与玻璃底荧光皿底部脱离,生物膜内结构紊乱。结论痤疮丙酸杆菌生物膜形成与其对抗菌药物耐药相关。

【Abstract】 Objective To analyze the correlation between biofilm formation and antibiotic resistance in Cutibacterium acnes (C.acnes) strains.Methods A total of 44 C.acnes strains were extracted and identified from lesions of patients with acne in Huashan Hospital,Fudan University from January 2019 to June 2019.Antimicrobial susceptibilities of the C.acnes strains were tested and interpreted in accordance with the CLSI 2018 breakpoints.Strains were cultivated for 96 h and 120 h in vitro,and subsequently stained with crystal violet dye to measure the biomass (D570nm) or resazurin dye to measure the viable bacteria count (560Ex/590Em).The biofilm structure was observed under a confocal laser scanning microscope (CLSM).Results After culture for 96 h and120 h,the biofilm biomass (D570nm) and viable bacteria count (560Ex/590Em) inside the biofilm were significantly higher in the antibiotic-resistant strains than in the sensitive strains (P < 0.05).CLSM showed the resistant strains adhered to the FluoroDish bottom,with a uniform membranous structure and an ordered internal biofilm structure.In contrast,the biofilms of the sensitive strains detached from the FluoroDish bottom with disordered internal structure.Conclusions Biofilm formation of C.acnes is correlated with antibiotic resistance.
