基因检测结核分枝杆菌异烟肼、利福平耐药与液体培养法对比的临床价值
Clinical Value Of Genetic Drug Resistance Detection Of Isoniazid And Rifampicin Of Mycobacterium Tuberculosis Compared With Liquid Culture Method
【摘要】 目的探讨基因检测结核分枝杆菌利福平、异烟肼耐药与液体培养法的临床效果。方法选择2018年2月至2019年7月我院收治的结核病患者脓液、灌洗液、脑脊液、胸腹腔积液以及痰液经培养证实为结核分支杆菌的180例阳性标本为研究对象,运用培养法和PCR溶解曲线法分别检测分离180例患者结核分枝杆菌菌株中的利福平和异烟肼耐药性,然后对荧光PCR溶解曲线法的特异度和灵敏度进行计算,并且比较两种方法的符合率。结果 PCR溶解曲线法检测异烟肼的特异度以及灵敏度分别为93.9%、85.4%;检测利福平特异度及灵敏度分别为97.1%、92.3%;但是两种方法的符合率比较无差异(P>0.05)。结论在结核分枝杆菌利福平、异烟肼耐药的检测中,与液体培养法相比,基因检测法具有准确率高、快速的优点,可以作为一种有效方法。
【Abstract】 Objective To explore clinical effect of genetic drug resistance detection of rifampicin and isoniazid of mycobacterium tuberculosis and liquid culture method.Methods We chose 180 cases of positive specimens of pus,lavage fluid,cerebrospinal fluid,pleural effusion and sputum of tuberculosis patients admitted to our hospital from February 2018 to July 2019 as research objects.With culture method and PCR melting curve method,detected rifampicin and isoniazid resistance of 180 patients with mycobacterium tuberculosis strains,calculated specificity and sensitivity of fluorescent PCR melting curve method,and compared compliance rate between two methods.Results Specificity and sensitivity of isoniazid detected by PCR melting curve method was 93.9% and 85.4% respectively;those of rifampicin were 97.1% and 92.3% respectively;there was no difference in coincidence rate of two methods (P>0.05).Conclusion In detection of rifampicin and isoniazid resistance of mycobacterium tuberculosis,compared with liquid culture method,gene detection method has advantages of higher accuracy and speed,which can be applied as an effective method.
