【摘要】 目的探讨联合检测宫颈长度及孕妇血清中细胞间黏附分子-1(ICAM-1)水平对早产的预测价值。方法选取2018年1月～2019年10月在我院产科规律产检并分娩的241例单胎妊娠产妇,根据产妇妊娠状态和分娩时孕龄分为健康对照组90例,先兆早产组88例,早产组63例。所有产妇行宫颈长度测量和血清ICAM-1浓度测定,比较三组产妇的上述指标水平,评价宫颈长度和ICAM-1对早产的预测价值。结果早产组宫颈长度短于健康对照组和先兆早产组,早产组血清ICAM-1水平高于另外两组,差异均有统计学意义(P<0.05);早产组宫颈长度阳性率和血清ICAM-1阳性率均高于健康对照组,差异均有统计学意义(P<0.05);宫颈长度联合血清ICAM-1的灵敏度和阴性预测值高于单一指标诊断。结论联合检测宫颈长度及孕妇血清中ICAM-1水平对早产发生有较高的预测价值,值得临床推广应用。更多还原

【Abstract】 Objective To explore the predictive value of cervical length and intercellular adhesion molecule-1(ICAM-1) in serum of pregnant women. Methods A total of 241 parturients with single pregnancy in our hospital from January2018 to October 2019 were selected. According to the pregnant state and the gestational age at delivery they were divided into healthy control group(n=90), threatened preterm delivery group(n=88) and preterm delivery group(n=63).The cervical length and serum ICAM-1 concentration were measured in all the parturients. The above indexes of the three groups were compared to evaluate the predictive value of cervical length and ICAM-1 on preterm delivery. Results the cervical length of preterm delivery group was shorter than that of healthy control group and threatened preterm delivery group, and the serum ICAM-1 level of preterm delivery group was higher than that of the other two groups, with statistical differences(P<0.05); the positive rate of cervical length and serum ICAM-1 in preterm delivery group was higher than that of healthy control group(P<0.05); the sensitivity and negative predictive value of cervical length combined with serum ICAM-1 were higher than those of single index diagnosis. Conclusion The combination of cervical length and serum ICAM-1 has a high predictive value for the occurrence of preterm delivery in pregnant women, which is worthy of clinical application.更多还原
