【摘要】 目的研究改良式经尿道前列腺等离子双极电切(Transurethral plasma bipolar resection of prostate,PKRP)术联合5α-还原酶抑制剂对伴中度下尿路症状良性前列腺增生(benign prostatic hyperplasia,BPH)患者的治疗效果。方法选取2018年6月～2019年6月我院收治伴中度下尿路症状BPH患者100例,按照随机非双盲方法将所有患者平均分为手术组和联合组各50例,手术组患者使用改良式PKRP术对患者进行治疗,联合组在手术组的基础上使用5α-还原酶抑制剂进行治疗,对两组患者手术情况、残余尿量、Qmax检测、夜尿次数、IPSS评分、生活质量评分、性功能指标、IIEF-5评分、治疗效果、并发症发生情况进行统计并比较。结果联合组手术时间、术中出血量、术后卧床时间、留置导尿管时间低于手术组,差异有统计学意义(P<0.05);联合组患者残余尿量、夜尿次数、IPSS评分以及并发症发生率均低于手术组,Qmax、生活质量评分、性功能指标以及治疗总有效率均高于手术组,差异有统计学意义(P<0.05)。结论改良式PKRP术联合5α-还原酶抑制剂治疗伴中度下尿路症状BPH患者疗效显著,能够有效改善患者前列腺症状和IPSS评分,改善性功能。更多还原

【Abstract】 Objective To investigate the therapeutic effect of modified transurethral plasma bipolar resection of prostate(PKRP) combined with 5α-reductase inhibitor inpatients with benign prostatic hyperplasia(BPH) with moderate lower urinary tract symptoms. Methods A total of 100 patients with BPH and moderate lower urinary tract symptoms who were admitted to our hospital from June 2018 to June 2019 were selected and divided into the operation group and the combined group according to random non-double-blind method, with 50 patients in each group. The operation group were treated with the modified PKRP procedure, and the combined group were treated with 5α-reductase inhibitors plus the treatment of the surgical group. The surgical condition, residual urine volume, Qmax, nocturnal frequency, IPSS score,quality of life score, sexual function index, IIEF-5 score, therapeutic effect and incidence of complications were analyzed statistically and compared. Results The operation time, intraoperative blood loss, postoperative bedtime and indwelling catheter time of the combined group were lower than those of the operation group, with statistically significant differences(P<0.05). The residual urine volume, nocturnal frequency, IPSS score and incidence of complications of the combined group were lower than those of the operation group, and the Qmax, quality of life score, sexual function indexand the total treatment effective rate of the combined group were higher than those of the operation group, with statistically significant differences(P<0.05). Conclusion Modified PKRP combined with 5α-reductase inhibitor is effective in the treatment of patients with BHP with moderate lower urinary tract symptoms. It can effectively improve prostate symptoms, IPSS score and sexual function of patients.更多还原
