【摘要】 目的探讨银翘散合麻杏石甘汤治疗慢性阻塞性肺疾病(COPD)急性加重期患者的临床效果。方法选取2015年3月～2018年3月浙江省嘉兴市中医医院收治的COPD急性加重期患者198例,按照随机数字表法将其分为对照组(n=99)和研究组(n=99)。对照组给予常规西医治疗,研究组在对照组的基础上联合银翘散合麻杏石甘汤治疗。比较两组临床疗效、中医证候积分、肺功能、炎性因子水平及不良反应发生率。结果两组治疗后中医证候积分均低于治疗前(P <0.05),且研究组低于对照组(P <0.05)。研究组治疗后的临床总有效率高于对照组(P <0.05)。两组治疗后用力肺活量(FVC)、第1秒用力呼气容积(FEV1)均大于治疗前,FEV1/FVC高于治疗前,且研究组改善情况优于对照组(P <0.05)。两组治疗后血清白介素-6、白三烯B4、肿瘤坏死因子-α水平均低于治疗前,且研究组低于对照组(P <0.05)。两组不良反应发生率比较,差异无统计学意义(P> 0.05)。结论银翘散合麻杏石甘汤联合西医常规治疗COPD急性加重期,可迅速改善患者临床症状,疗效显著,可有效改善患者肺功能,降低机体炎性因子水平,用药安全性较好,临床应用价值较高。更多还原

【Abstract】 Objective To investigate clinical efficacy of Yinqiao Powder combined with Maxing Shigan Decoction in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease（COPD）. Methods One hundred and ninety-eight patients with acute exacerbation of COPD admitted to Jiaxing Hospital of Traditional Chinese Medicine（TCM） in Zhejiang Province from March 2015 to March 2018 were selected, they were divided into control group（n = 99） and study group（n = 99） according to the random number table method. Control group was treated with conventional Western medicine. Study group was treated with Yinqiao Powder and Maxing Shigan Decoction on the basis of control group. Clinical efficacy, TCM symptom score, pulmonary function, levels of inflammatory factors and incidence of adverse reactions were compared between two groups. Results After treatment, the scores of TCM syndromes in two groups were lower than those before treatment（P < 0.05）, and those in study group were lower than those in control group（P < 0.05）. Total clinical effective rate of study group was higher than that of control group（P < 0.05）. After treatment, forced vital capacity（FVC） and forced expiratory volume in the first second（FEV₁） of two groups were higher than before treatment, FEV₁/FVC of two groups was higher than before treatment, and the improvement of study group was better than that of control group（P < 0.05）. After treatment, the serum levels of interleukin-6, leukotrienes B4 and tumor necrosis factor-α in two groups were lower than those before treatment, and study group was lower than control group（P < 0.05）. There was no significant difference in the incidence of adverse reactions between two groups（P > 0.05）. Conclusion Yinqiao Powder and Maxing Shigan Decoction combined with routine Western medicine in the treatment of acute exacerbation of COPD can rapidly improve the clinical symptoms of patients with significant curative effect, effectively improve the pulmonary function of patients, reduce the level of inflammatory factors, drug safety is better, clinical application value is higher.更多还原
