定量脑电图对模式经颅磁刺激治疗抑郁症疗效预测
Efficacy predict of mode transcranial magnetic stimulation by quantitative electroencephalogram in the treatment of depression
【摘要】 目的探讨定量脑电图对模式经颅磁刺激治疗抑郁症的疗效预测。方法选取60例抑郁症患者,分为研究组(常规治疗+模式经颅磁刺激)和对照组(常规治疗)各30例。予以两组定量脑电图检查,比较两组(δ+θ)/(α+β)相对功率比。结果两组治疗后HAMD评分均较各自治疗前降低(P<0.05),研究组治疗后HAMD评分低于对照组(P<0.05)。两组治疗后F3、FP1、C3、T3、F7频段(δ+θ)/(α+β)值均较各自治疗前下降(P<0.05),其中研究组治疗后F3频段较对照组下降(P<0.05)。Pearson相关性分析显示,研究组定量脑电图F3、FP1、C3、T3、F7频段(δ+θ)/(α+β)值与HAMD评分呈正相关(P<0.05)。结论定量脑电图F3、FP1、C3、T3、F7频段(δ+θ)/(α+β)值能够提示模式经颅磁刺激治疗抑郁症后左侧大脑皮质神经电活动变化,其中以F3频段最为敏感,可作为神经电生理学有效预测指标推广使用。
【Abstract】 Objective To explore the predictive effect of quantitative electroencephalogram(QEEG) on the treatment of depression by mode transcranial magnetic stimulation. Methods A total of 60 patients with depression were selected and randomly divided into study group(routine treatment & mode transcranial magnetic stimulation, 30 cases) and control group(routine treatment, 30 cases). QEEG was performed in both groups, and the relative power ratio of two groups(δ+θ)/(α+β) was compared.Results After treatment, the total scores of HAMD in both groups decreased significantly compared with those before treatment(P<0.05), and the total score of HAMD in study group was significantly lower than that in control group(P<0.05). After treatment, the five frequency bands(δ+θ)/(α+β) of F3, FP1, C3, T3 and F7 all decreased significantly compared with those before treatment(P<0.05), and the F3 frequency band(δ+θ)/(α+β) in study group was significantly lower than that in control group(P<0.05). Pearson correlation analysis showed that the five frequency bands(δ+θ)/(α+β) of the QEEG F3, FP1, C3, T3 and F7 were positively correlated with the total score of HAMD(P<0.05).Conclusion QEEG F3, FP1, C3, T3 and F7 bands(δ+θ)/(α+β) can indicate the changes of left cerebral cortex neuroelectric activity after transcranial magnetic stimulation for depression, among which F3 is the most sensitive and can be used as an effective predictor of neuroelectrophysiology.
