【摘要】 报道1例以意识障碍伴双侧针尖样瞳孔起病,意识转清后遗留情感、言语、认知障碍及垂直眼肌麻痹的Percheron动脉闭塞患者,复查头部扩散加权成像(DWI)显示双侧丘脑对称性高信号,无中脑受累。Percheron动脉闭塞患者常以意识障碍为首发表现,首次头部MRI可为阴性,易延误诊断与后续治疗。因此,首次头部DWI阴性应考虑Percheron动脉闭塞的诊断,临床医师应增强对Percheron动脉闭塞致双侧丘脑梗死的早期识别及干预。更多还原

【Abstract】 The paper presented a case of artery of Percheron occlusion with acute onset of consciousness disorder accompanied by bilateral pinpoint pupils,left with altered emotional status,speech disorder,cognitive impairment and vertical ophthalmoplegia after regaining consciousness. Reexamination with duffusion weighted imaging( DWI) of the head showed symmetrical high intensity signals in bilateral thalamus without midbrain involvement. In patients with artery of Percheron occlusion,consciousness disorder is often the first manifestation,and the first head magnetic resonance imaging examination may be negative,which is likely to delay the diagnosis and subsequent treatment. Therefore,the diagnosis of artery of Percheron occlusion should be considered when encountering negative results in the initial head DWI,and clinicians should enhance the early recognition and intervention of bilateral thalamic infarction caused by artery of Percheron occlusion.更多还原
