多囊卵巢综合征患者经阴道超声检测子宫内膜结构和机械运动对人工授精妊娠结局的价值
Value of vaginal ultrasound to detect endometrial structure and endometrial mechanical movement in women with polycystic ovary syndrome who have undergone artificial insemination
【摘要】 目的探讨多囊卵巢综合征患者应用阴道超声检测子宫内膜结构和机械运动对人工授精妊娠结局的价值。方法收集2018年8月至2019年12月在漯河市中心医院接受人工授精(IUI)治疗的多囊卵巢综合征(PCOS)不孕妇女共201例,通过来曲唑促排卵,其中189例有排卵行人工授精,根据是否妊娠分为妊娠组(33例)和未妊娠组(156例),通过阴道超声观察两组妇女排卵前子宫内膜蠕动波、子宫内膜厚度(A、B、C型)、子宫动脉阻力指数(RI)、子宫动脉搏动指数(PI)等指标来比较子宫内膜结构和子宫内膜的机械运动来预测AIH的结局。结果妊娠组排卵前的子宫内膜厚度和子宫内膜形态与未妊娠组比较,差异未见统计学意义(P> 0.05),但是排卵前子宫内膜厚度高于7 mm,以A型、B型为主;两组间排卵期RI、PI比较,差异有统计学意义(P <0.05);妊娠组子宫内膜蠕动波正向运动高于未妊娠组,子宫内膜蠕动波的频率低于未妊娠组。结论子宫内膜的厚度及分型结合子宫内膜蠕动波的方向及频率与胚胎的着床密切相关,在多囊卵巢综合征患者中,应用阴道超声检查子宫内膜结构和机械运动对人工授精的妊娠结局有一定的应用价值。
【Abstract】 Objective To investigate the value of vaginal ultrasound to detect endometrial structure and endometrial mechanical movement in women with polycystic ovary syndrome who have undergone Artificial insemination(AIH).Methods A total of 201 cases infertile women with Polycystic ovary syndrome who have undergone artificial insemination in the Central Hospital of Luohe were collected.Letrozole was used for ovulation induction,of which 189 cases had ovulation and artificial insemination.They were divided into pregnancy group(33 cases) and non-pregnancy group(156 cases)according to whether they were pregnant.The endometrial peristaltic wave,endometrial thickness(types A,B,C),uterine spiral arterial blood flow index(PI,RI) and other indicators of the two groups before ovulation were observed by vaginal ultrasound, in order to compare the endometrial structure and endometrial mechanical movement to predict the outcome of AIH.Results There was no significant difference in endometrial thickness or morphology before ovulation between pregnancy group and non-pregnancy group(P > 0.05),but the thickness of the endometrium before ovulation was more than 7 mm,mainly type A and type B.There were significant differences in uterine spiral arterial index(RI,PI)during ovulation between the two groups(P < 0.05).The positive movement of endometrial peristaltic wave in pregnancy group was higher than that in non-pregnancy group,and the frequency of endometrial peristaltic wave was lower than that in non-pregnant group pregnancy group.Conclusions Endometrial thickness and endometrial type combined with the direction and frequency of endometrial peristaltic waves are closely correlated with embryo implantation,vaginal ultrasound examination of endometrial structure and mechanical movement can be used for artificial insemination in patients with polycystic ovary syndrome.The pregnancy outcome has certain application value.
