不同妊娠期孕妇血清甲状腺激素与自身抗体水平变化及其相关性分析

【摘要】 目的探讨不同妊娠期孕妇血清甲状腺激素及自身抗体的动态水平变化及相关性。方法采用放射免疫法和化学发光法对110例不同妊娠期孕妇和100例非妊娠正常育龄妇女(对照组)甲状腺激素及自身抗体含量测定。结果整个孕期总甲状腺素(TT3)和促甲状腺激素(TSH)有增高趋势(P <0. 01),早期TSH水平最低,之后逐渐回升,最终晚期到达最高水平,总三碘甲状腺原氨酸(TT4)在早孕阶段升高,中孕达高峰,逐渐下降。整个孕期TSH水平与TT4水平负相关(r=-0. 25,P <0. 01),TSH水平与TT3水平无相关性,除了早孕TSH水平与晚孕甲状腺过氧化物酶抗体有相关性(r=0. 27,P <0. 01)外,孕妇的甲状腺激素与自身抗体之间无相关性; TT3与TT4在早孕、中孕、晚孕期间有很好的相关(r=0. 57,P <0. 01; r=0. 31,P <0. 01; r=0. 33,P <0. 01)。结论在妊娠期间,加强监测孕妇的各甲状腺激素动态变化,对正常妊娠和胎儿健康发育及临床治疗有重要意义。

【Abstract】 Objective To investigate the dynamic changes and relationship of serum thyroid hormone and autoantibody in pregnant women with different gestational periods. Methods The contents of thyroid hormone and autoantibody were determined in 110 pregnant women and 100 non-prednant women of normal reproductive age by radioimmunoassay and chemilumine scence assay.Results Triiodothyronine( TT3) and Thyroidstimulating hormone( TSH) increased in the whole pregnancy( P < 0. 01),the lowest TSH level in the early stage,then gradually increased,and finally reached the highest level in the late stage. Thyroxin( TT4) increased in the early stage,reached the peak in the middle stage,and gradually decreased. There was negative correlation of TSH level with TT4 level in pregnancy( r =-0. 25,P < 0. 01),but there was no correlation of TSH level with TT3 level. In addition to the correlation of TSH level in early pregnancy with TPOAb in late pregnancy( r = 0. 27,P < 0. 01),there was no correlation of thyroid hormone with autoantibody in pregnant women; there was good correlation of TT3 with TT4 in early,middle and late pregnancy( r = 0. 57,P < 0. 01; r = 0. 31,P < 0. 01; r = 0. 33,P < 0. 01). Conclusions Thyroid hormone normal references during pregnancy offer a great clinical tool for diagnosis and monitoring for pregnancy and fetal health.
