女性生殖道感染对宫颈癌和宫颈上皮内瘤变的影响
Effect of female genital tract infection on cervical cancer and cervical intraepithelial neoplasia
【摘要】 目的探讨女性生殖道感染对宫颈癌和宫颈上皮内瘤变(CIN)的影响。方法选取2016年1月~2019年1月我院收治的147例CIN或宫颈癌患者作为研究组,选取同期就诊的52例非CIN和宫颈癌的患者作为对照组。根据患者病变程度的不同,将研究组分为CINⅠ组(32例)、CINⅡ组(43例)、CINⅢ组(48例)和宫颈癌组(24例)。比较研究组中CIN和宫颈癌患者的分布构成比;比较各组的细菌性阴道炎(BV)、沙眼衣原体(CT)和人乳头瘤病毒(HPV)感染率。分析生殖道感染对宫颈癌和CIN变的影响。结果 147例CIN和宫颈癌患者中主要为CINⅡ和CINⅢ,分别占研究组总比的29.25%和32.65%。研究组的BV、CT和HPV感染率均高于对照组,差异有统计学意义(P<0.05)。宫颈癌组的BV、CT感染率高于CINⅠ、CINⅡ和CINⅢ组,差异有统计学意义(P<0.05);CINⅢ和宫颈癌组的HPV感染率高于CINⅠ组和CINⅡ组,差异有统计学意义(P<0.05);CINⅠ、CINⅡ和CINⅢ组的BV、CT感染率比较,差异无统计学意义(P>0.05);CINⅠ与CINⅡ组的HPV感染率比较,差异无统计学意义(P>0.05)。结论女性生殖道内BV、CT和HPV感染是导致宫颈内良性和恶性瘤变发生的重要诱因,加强检查,及时诊治,有效控制患者病情的进展,对预防宫颈癌变、降低宫颈癌死亡率具有重要意义。
【Abstract】 Objective To investigate the effect of female genital tract infection on cervical cancer and cervical intraepithelial neoplasia(CIN). Methods A total of 147 patients with CIN or cervical cancer admitted to our hospital from January 2016 to January 2019 were enrolled as the study group, a total of 52 patients with non-CIN and cervical cancer who were treated at the same time were selected as the control group. Depending on the degree of patients with lesions, the team can be divided into CIN Ⅰ group(32 cases), CIN Ⅱ group(43 cases), CIN Ⅲ group(48 cases) and cervical cancer group(24 cases). The distribution ratios of CIN and cervical cancer patients in the study group were compared; the bacterial vaginitis(BV), Chlamydia trachomatis(CT) and human papillomavirus(HPV) infection rates were compared between the groups. The effect of genital tract infections on cervical cancer and CIN was analyzed. Results Among the 147 patients with CIN and cervical cancer, CIN Ⅱ and CIN Ⅲ were the main ones, accounted for29.25% and 32.65% of the study group, respectively. The BV, CT and HPV infection rates of the study group were higher than the control group, and the differences were statistically significant(P<0.05). The BV and CT infection rates in the cervical cancer group were higher than those in the CIN Ⅰ, CIN Ⅱ and CIN Ⅲ groups, and the differences were statistically significant(P<0.05). The HPV infection rate in CIN Ⅲ and cervical cancer group were higher than that in CIN Ⅰ group and CIN Ⅱ group, and the differences were statistically significant(P<0.05). There were no difference in BV and CT infection rates between CIN Ⅰ, CIN Ⅱ and CIN Ⅲ groups. There were no significant difference in the HPV infection rate between CIN Ⅰ and CIN Ⅱ group(P>0.05). Conclusion BV, CT and HPV infection in the female reproductive tract is an important cause of benign and malignant neoplasia in the cervix. Strengthening the examination, timely diagnosis and treatment, and effectively controlling the progress of the patient′s condition are of great significance for preventing cervical cancer and reducing cervical cancer mortality.
