原发性高血压患者合并非酒精性脂肪肝与脂联素及胰岛素抵抗的相关性分析

【摘要】 目的探讨原发性高血压患者合并非酒精性脂肪肝(NAFLD)与脂联素(APN)及胰岛素抵抗(IR)的关系。方法选取2015年5月~2016年6月潍坊市寒亭区人民医院收治的67例原发性高血压患者作为研究对象,根据是否合并有NAFLD分为脂肪肝组(n=33)和非脂肪肝组(n=34)。比较两组患者的体重指数(BMI)、空腹血糖(FBG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、谷丙转氨酶(ALT)等指标水平,用酶联免疫吸附法(ELISA)测定法检测两组患者血清APN浓度,放免法测定空腹胰岛素(FINS),采用稳态模型(HOMA-IR)法评估IR,并进行比较。结果脂肪肝组的BMI、FBG、TC、TG、LDH-C、ALT、FINS、HOMA-IR均高于非脂肪肝组,而脂肪肝组的HDL-C、APN水平低于非脂肪肝组,差异有统计学意义(P<0.05)。APN与SBP、DBP、TG、HbA1c、FINS、HOMA-IR均成负相关(r=-0.48、-0.36、-0.44、-0.31、-0.32、-0.34,P<0.05)。结论原发性高血压患者合并NAFLD与APN及IR密切相关。

【Abstract】 Objective To investigate the relationship among insulin resistance, adiponectin(APN) and non-alcoholic fatty liver disease(NAFLD) in patients with essential hypertension(EH). Methods A total of 67 patients with EH treated in Weifang Hanting District People′s Hospital from May 2015 to June 2016 were selected as the subjects, and divided into EH+NAFLD group(with NAFLD, n=33) and EH group(without NAFLD, n=34). The indicators including body mass index(BMI), fasting blood glucose(FBG),total cholesterol(TC), triacylglycerol(TG), low density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol(HDL-C), alanine aminotransferase(ALT) were compared between the two groups. The APN was determined by enzyme-linked immunosorbentassay(ELISA), fasting insulin(FINS)were determined by radioimmunoassay, and IR was assessed by homeostasis model assessment-insulin resistance(HOMA-IR) test. Results The levels of BMI, FBG, TC, TG, LDH-CH, ALT, FINS, HOMA-IR in EH+NAFLD group were higher than those in EH group, and the levels of HDL-C and APN in EH+NAFLD group were lower than those in EH group(P<0.05). The level of ANP was negatively correlated with SBP, DBP, TG, HbA1c, FINS and HOMA-IR in patients with EH(r=-0.48,-0.36,-0.44,-0.31,-0.32,-0.34, P<0.05). Conclusion NAFLD in patients with EH is closely related to APN and Insulin resistance.
