大脑中动脉穿支供血区单发脑梗死CISS临床分型及LDL-C、超敏-C反应蛋白的关系

【摘要】 目的观察大脑中动脉穿支供血区单发脑梗死依据CISS分型情况,及LDL-C、超敏-C反应蛋白水平与不同分型间的关系。方法连续入选100例急性大脑中动脉穿支供血区单发脑梗死患者,依据CISS分型进行二次分型,同时测定LDL-C、超敏-C反应蛋白水平,比较不同分型组间的LDL-C、超敏-C反应蛋白水平。结果根据CISS分型二次分型后,穿支动脉病变型63例,大动脉粥样硬化型37例,大动脉粥样硬化型比例为37%。其中穿支动脉型组LDL-C水平为(3. 08±0. 92) mmol/L,大动脉粥样硬化型组LDL-C水平为(3. 50±0. 88) mmol/L,存在统计学差异(P <0. 05);穿支动脉型组超敏-C反应蛋白水平为(3. 36±2. 30) mmol/L,大动脉粥样硬化型组超敏-C反应蛋白水平为(6. 85±2. 08) mmol/L,存在统计学差异(P <0. 05)。结论依据CISS分型大动脉粥样硬化型LDL-C及超敏-C反应蛋白水平均高于穿支动脉型,二次分型为大动脉粥样硬化型患者无论LDL-C水平如何均应强化调脂治疗,临床工作中应更广泛的采取CISS病因分型。

【Abstract】 Objective To analyze the classification of infarct of perforating arteries which from the middle cerebral artery( MCA) perforating artery based on Chinese Ischemic Stroke Subclassification( CISS),and further to investigate the relationship between low density lipoprotein LDL-C level,high sensitive C-reactive protein( Hs-CRP) level and the classfication based on CISS. Methods Continuous selected 100 cases of acute middle cerebral artery perforators single-shot cerebral infarction patients with blood supply area,according to the secondary classification of CISS parting,and test LDL-C,high-sensitivity-C reactive protein levels,compare levels of different points between groups of LDL-C,hypersensitive creactive protein.Results According to the second classification of the patients based on CISS,there were 63 large-artery atherosclerosis( LAA) and 37 perforating artery disease( PAD). The percentage of LAA was 37%. The LDL-C level in perforating artery disease were( 3. 08 ± 0. 92) mmol/L,and in large-artery atherosclerosis were( 3. 50 ± 0. 88) mmol/L respectively. The statistical comparison between the two groups is significant( P < 0. 05). The Hs-CRP level in perforating artery disease were( 3. 36 ±2. 30) mmol/L,in the large-artery atherosclerosis were( 6. 85 ± 2. 08) mmol/L,between the two groups there was a significant difference( P < 0. 05). Conclusion According to CISS classification,the LDL-C and Hs-CRP levels of LAA were higher than the levels of PAD. If the second classification of the patients was LAA,the patients need to strengthen lipid-lowering therapy whether the LDL-C level is high or normal. The CISS should be more widely adopted in clinical work.
