MSCT延迟扫描对颅内结核患者的临床诊断价值

【摘要】 目的探讨多层螺旋CT(MSCT)延迟扫描对颅内结核患者的临床诊断价值。方法选择我院2017年5月至2018年7月收治的43例颅内结核患者作为研究对象,患者均给予16层螺旋CT常规扫描,随后注入对比剂进行螺旋扫描,将其结果作为动脉期结果;延迟5 min后再次进行扫描,将其结果作为延迟期结果。观察所有患者的诊断结果。结果对43例颅内结核患者进行检查,共发现病灶579个,其中脑膜增厚29个,脑膜结核瘤266个,脑实质结核瘤284个。延迟期脑膜增厚、脑膜结核瘤、脑实质结核瘤的病灶大小、病灶周边清晰度评分均高于动脉期(P<0.05);延迟期脑膜增厚、脑膜结核瘤、脑实质结核瘤的血管断面辨别能力评分均高于动脉期(P<0.05);延迟期脑膜增厚、脑膜结核瘤、脑实质结核瘤病灶与血管断面容易辨别的占比高于动脉期(P<0.05)。结论 MSCT延迟扫描运用于颅内结核中具有重要的诊断价值,不仅可提升病灶图像质量,同时有助于对病灶与血管断面进行辨别,具有推广及应用的价值。

【Abstract】 Objective To explore the clinical diagnosis value of multi-slice spiral CT(MSCT) delayed scanning for patients with intracranial tuberculosis. Methods A total of 43 patients with intracranial tuberculosis in our hospital from May 2017 to July 2018 were selected as the research objects. Routine 16-slice spiral CT scans were performed in all patients, then contrast medium was injected for spiral scanning, the results of which were taken as arterial phase results,and repeated scanning after 5 min, the results of which were taken as delay phase results; the diagnostic results of all patients were observed. Results A total of 43 patients with intracranial tuberculosis were examined, among them, there were579 lesions, including 29 cases of meningeal thickening, 266 cases of meningeal tuberculoma and 284 cases of parenchymal tuberculoma. The size and scores of peripheral clarity of lesions of meningeal thickening, meningeal tuberculoma and parenchymal tuberculoma in the delay phase were higher than those in the arterial phase(P<0.05); the identification ability scores of blood vessel section surface of meningeal thickening, meningeal tuberculoma and parenchymal tuberculoma in the delay phase were higher than those in the arterial phase(P<0.05); the proportions of lesions easily distinguished from blood vessel section surface of meningeal thickening, meningeal tuberculoma and parenchymal tuberculoma in the delay phase were higher than those in the arterial phase(P<0.05). Conclusion MSCT delayed scanning has important diagnostic value in intracranial tuberculosis, it can not only improve the image quality of the lesion, but also help to distinguish the lesion from the vascular section, which has the value of popularization and application.
