AdeB基因变异介导鲍曼不动杆菌多药耐药
AdeB gene mutation mediated Acinetobacter baumannii multidrug resistance
【摘要】 采用纸片扩散法(K-B法)测定鲍曼不动杆菌(AB)菌株对临床常用9大类22种抗生素的耐药性,应用PCR检测耐药基因,将耐药基因adeB转化至敏感菌株AB43中,荧光定量PCR检测AdeB mRNA的转录,药敏试验检测转化后菌株的耐药性变化。结果表明:收集的68株AB对亚胺培南和美罗培南耐药率分别高达51.47%和50.00%。耐药基因adeR、adeS、adeA、adeB、adeC、OXA-23、OXA-51、TEM-1、VIM-1在碳青霉烯类耐药、中介及敏感株中的分布差异不大;adeB基因在耐药株和敏感株中的序列存在较大差异,二者同源区也存在3个氨基酸残基的变异;耐药菌株来源的adeB基因在敏感菌株中表达后可导致AB的耐药性显著增强。这一研究提示AdeB基因变异可导致AB多药耐药性显著增加。
【Abstract】 The paper diffusion method(K-B method) was used to determine the resistance of A. baumannii strains to 22 kinds of antibiotics in 9 clinical categories. PCR was used to detect drug resistance genes, and adeB was transformed into sensitive strain AB43. Quantitative Real-time PCR was used to detect the transcription of AdeB mRNA, and susceptibility test was used to detect the drug resistance of transformed strains. The results showed that the resistance rates of 68 A. baumannii strains collected by our group to imipenem and meropenem were as high as 51.47% and 50%, respectively. The distribution of drug resistance genes adeR, adeS, adeA, adeB, adeC, OXA-23, OXA-51, TEM-1, VIM-1 in carbapenem-resistant, intermediate and sensitive strains were not different significantly. There were large differences in the sequences of adeB genes from resistant and susceptible strains; three amino acid residues were found in the homologous regions of the deduced AdeB. The expression of the adeB gene derived from the resistant strain in the sensitive strain can lead to a significant increase in the drug resistance of Acinetobacter baumannii. These data indicated that adeB gene mutation could lead to multidrug resistance of A. baumannii.
