【摘要】 目的:评估中药烫疗治疗社区腰椎间盘突出症(LDH)疼痛的安全性、有效性及患者的接受程度。方法:应用中药烫疗治疗LDH患者684例。采用视觉模拟评分法(VAS)评估患者治疗前后腰痛及其对生活质量的影响程度,应用Logistic回归分析疗效影响因素。结果:止痛总有效率为77.2%。治疗前重度疼痛和病程≤6个月总有效率(91.8%、90.9%)显著高于轻中度疼痛(70.7%)和病程>6个月(76.1%)。腰痛程度(OR=4.877)越严重,腰痛病程(OR=0.256)越短,疗效越好。对日常生活、日常工作和行走能力等总有效改善率分别为79.7%、80.2%、80.3%。不良反应发生率为3.5%。患者对技术治疗效果满意度和认可度分别为84.0%和86.3%。结论:中药烫疗治疗LDH疼痛安全有效、患者依从性较好、接受度较高,且技术易于被基层医生掌握,适合在基层医疗机构推广应用。更多还原

【Abstract】 Objective: To evaluate the safety, efficacy and acceptability of burn treatment with traditional Chinese medicine in treating the pain of lumbar intervertebral disc herniation(LDH) in community. Methods: A total of 684 cases of LDH were treated with traditional Chinese medicine burn therapy. Visual analogue scoring(VAS) was used to record the effect of pain and lumbago on the quality of life of patients before and after treatment, and logistic regression was used to analyze the influence factors of pain effect. Results: The total effective rate of analgesia was 77.2%. The total efficiency in the treatment of severe pain group and course ≤6 months(91.8%, 90.9%) was significantly higher than that in the mild and moderate pain group(70.7%) and the course of disease >6 months(76.1%). The more severe the lower back pain is, the better the curative effect(OR=4.877), and the shorter the course of lumbago is the better(OR=0.256). The overall effective rate of improvement is 79.7%, 80.2% and 80.3%, respectively on the effects of daily life, daily work and walking ability. The i ncidence of adverse reactions was 3.5%. The patients’ satisfaction and approval degree of technical treatment were 84.0% and 86.3%. Conclusions: The treatment with TCM burn therapy of LDH pain is safe and effective,and patient compliance is better; degree of acceptance is high, and the technology is easy to be mastered by grassroots doctors, then suitable for promotion in grass-roots.更多还原
