心肌缺血再灌注损伤患者血清TNF-α和IL-10水平变化及意义

【摘要】 目的探讨心肌缺血再灌注损伤患者血清肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)和白细胞介素-10(interleukin-10,IL-10)水平变化及意义。方法将2015年11月至2016年6月黑龙江省医院收治的急性心肌梗死行经皮冠状动脉介入治疗后发生缺血再灌注损伤的60例患者纳入研究组,将该院同期60例健康体检者纳入对照组。分别采集两组研究对象的空腹静脉血,检测血清TNF-α、IL-10水平并进行比较和分析。结果再灌注后1、6、12、24小时,研究组患者血清TNF-α水平均明显高于对照组(P<0.05),IL-10水平均明显低于对照组(P<0.05);随着再灌注时间的延长,研究组患者血清TNF-α水平呈上升趋势(P<0.05),而IL-10水平呈下降趋势(P<0.05)。研究组不同心功能分级患者血清TNF-α水平均明显高于对照组(P<0.05),IL-10水平均明显低于对照组(P<0.05),且心功能分级越高,TNF-α水平越高(P<0.05),IL-10水平越低(P<0.05)。结论 TNF-α和IL-10在心肌缺血再灌注损伤的发生、发展中具有重要作用,可作为评价心肌再灌注损伤程度的指标。

【Abstract】 Objective To study the changes of serum tumor necrosis factor-α(TNF-α) and interleukin-10(IL-10) in patients with myocardial ischemia reperfusion injury and its clinical significance. Method From November 2015 to June 2016, 60 patients with acute myocardial infarction underwent percutaneous coronary intervention in Heilongjiang Provincial Hospital were selected as study group; during the same period, 60 cases of healthy examiners were selected as control group. The fasting venous blood of the two groups were extracted, then serum levels of TNF-α and IL-10 were measured and compared. Result The level of serum TNF-αin study group was significantly higher than that in control group at 1 h, 6 h, 12 h and 24 h after reperfusion(P < 0.05), while the level of IL-10 was lower than that in control group(P < 0.05); with the prolongation of reperfusion time, the level of serum TNF-αin study group was increased(P < 0.05), while the level of IL-10 decreased(P < 0.05). The level of serum TNF-α in patients with different cardiac function was significantly higher than that in control group(P < 0.05), while the level of IL-10 was lower than that in control group(P < 0.05), and the higher classification of cardiac function, the higher the level of TNF-α(P < 0.05), the lower the IL-10 level(P < 0.05). Conclusion TNF-α and IL-10 play an important role in the development and progression of myocardial ischemia reperfusion injury and can be used to evaluate the severity of myocardial ischemia reperfusion injury.
